Yes, it is still reproducible in latest Kate 22.04.1 in Windows, with both
Courier and Courier New fonts. When you press Ctrl + Scroll the mouse wheel,
you could easily notice that on some font sizes the underscores disappear.

SUMMARY
When the search string entered in the "Search and Replace" plugin is a regular
expression with lookahead or lookbehind part, the replace functionality does
not work.

STEPS TO REPRODUCE
1. Open Kate and write a simple text in the editor, for ex:
SRC0
SRC1
SRC2

2. Open Search and Replace, enable regular expression search and type the
following regex in the search field:
SRC(?=\d)

3. Type any replace string in the replace field, for ex:
DST

3. Press Search. The found lines will display fine.

4. Press Replace or Replace Checked

OBSERVED RESULT
The found "SRC" strings are not replaced with "DST"

EXPECTED RESULT
The found "SRC" strings should be replaced with "DST"

SOFTWARE/OS VERSIONS
Observed in Kate 21.12.0 for Windows, as well as Kate in the unstable branch of
KDE Neon.
Qt Version: 5.15.2 (for the Windows version)

ADDITIONAL INFORMATION
In KWrite (in KDE Neon unstable branch), the above steps work fine.

I'm marking back the issue as REPORTED, since it is not clear if the big is
solved or not. I mean, the behavior have changed, but it is not clear if it is
correct. Please see my previous comment.

I checked with the latest official windows release I have 21.04.0 (present in
Microsoft Store), but the problem seems not solved.

Since the official windows release is rather old one, I downloaded Kate
21.11.70 nightly (for win64), and the behavior is slightly different:
I have mixed text in both Bulgarian and English languages, and Automatic Spell
check enabled. It looks the spell check for both languages work simultaneously
somehow. I mean, it does not matter which language is selected, the
correct/known words in both languages are not red underlined, and
incorrect/unknown words (also in both languages) are red underlined.

I'm not sure if that is intended and if it should work that way. If yes, then
the bug is fixed. However, if that is not intended and the spell checking
should be performed only for one selected language, then this probably
escalated into another issue.

It would be good if someone could comment that behavior.

In addition to the last comments, the (only) ways to enable/apply sub-pixel
anti-aliasing is either by cycling through sub-pixel rendering options
RGB->None->RGB (with pressing Apply after each step) or by disabling and then
enabling Anti-aliasing (again with Apply after each step). It cannot be applied
by changing the Hinting options.
I also confirm that sub-pixel anti-aliasing is not applied by default, although
it is set in Fonts settings.
The above behavior is observed with neon-developer-20210628-1846.iso image.
It looks the issue is partially fixed.

Yes, changing the default font (Courier New) to other one, resolves the issue.
Also changing the font size resolves it. However, with Courier New there are
certain other sizes that causes the underscores to disappear or to be displayed
with incorrect thickness (too thin).

Missing underscores in Ruby constant

Underscores are not visible on text elements with boldface decoration (for ex.
