It's possible that I was not running KPXC some of the times I had tried. It seems that neochat provides no indication when the secret service is not running. I do think that printing a bit more information to stdout would help. Especially when the secret service is not running. I'll close this though, since this seems to be resolved. I restarted neochat and after a third time it did work; I got two successive requests from keepassxc to disclose secrets (looks like neochat tried to read them multiple times at startup this time?) and then it worked.
