All 3 actions work fine now in the latest Okular version.

The bug is still in Okular 1.11 (tested on Windows using the nightly build
585).

The improvement is that the button to show the form in fullscreen works now,
the button to reset the form and to print it, are still not working.

Reading this again, the fact that it works in Evince--which also uses
Poppler--implies that the issue is in Okular.

I would assume that both Okular and poppler will need additional work here, but

Confirmed with Okular from git master and Poppler 0.6.0 (also from git master).

Albert or Oliver, is this a problem with Okular, or Poppler?

The program Evince supports at least to print the PDF.
