Delete kdeglobals off your .config folder, then delete the .cache folder, log out then log in. Finally the previews are by default on always anytime I open the file picker. Perfectly describes my issue too. Has been happening since I updated my KDE installment to 5.27.1 (latest as of this comment). For reference, I tried changing it off ~./.config/xdg-desktop-portal-kderc and putting in the string "Show Inline Previews=true", yet no effect. Unfortunately, not tech-savvy enough to understand these terms, but if this is any help, it's the file dialog off any program that calls off the KDE file chooser when you want to upload into or save a image off, like Firefox. Interestingly enough, this bug seems to have been resurfaced.
