After multiple update-initramfs and kernel reinstalls instead of message 'fstype not found' I have 'can't open /dev/mapper/ubuntu-root no such file'. However all the rest of the stacktrace seems to be similar as already attached photo of the stacktrace. I check if I have installed lvm2 and it is. I tried to use previous kernel, then reinstall kernels but all that didn't help me. I have luks encrypted partition which contains multiple lvm with ext4 volumes. This was working since 14.04 was released so this not standard configuration should not cause this issue. I am able to run live cd, then decrypt luks partition and mount volumes and chroot. All seems to be ok. I fsck lvm and all is ok. I found similar issue https://bugs.launchpad.net/ubuntu/+source/cryptsetup/+bug/1273261 which suggest to try udev_settle. This didn't helped. I even entered sleep 5 (thus "sleeping for 5 secs" message on the stacktrace in the middle) but this didn't solve the issue. Maybe I enter this hack too early in the script? No more idea... so mayday, mayday... I attached photo of the stacktrace and report of 'apport-cli linux-generic'. On 12.04, I've seen three possible failure scenarios (and so far, no success completing the test run): 1- The test runs for 2 hours but upon finishing it exits with signal 9. 2- The test runs for less than 2 hours, exiting because some of the workers got signal 9. 3- The system freezes before 2 hours have elapsed, requiring a reboot. This used to be a problem with early Unity (v5/v6?) doing I/O bound operations in threads used for rendering. Which is obviously bad. Maybe we haven't fully solved that yet or regressed. Also, the choice of filesystem formats used on the USB drive could be a factor. In Ubuntu/Gnome/Nautilus I think the formats NTFS and ExFAT for example are handled in userspace via GVFS [1]. So try some USB drives that are ext2/ext3/ext4/FAT32 to compare. [1] https://en.wikipedia.org/wiki/GVfs Either way, there's no excuse for this bug. After installing Xenial [0] I experience the following: when I do I/O through USB (to/from simple pendrives, or an external HDD I have), the desktop becomes unusable, because it goes very very unresponsive. With unresponsive I mean: - all windows go greyed out (like when it's failing to answer in time, as if the process inside that window locked up) - firefox and thunderbird start to pop up the typical "the script is taking too much to finis, want to kill it/continue/etc" (in whatever tab I'll be, so it's not the JS scripts (for once!) - the mouse pointer goes jumpy, not really following my hand movement - I see in thee indicator-multiload widget that CPU time is "wasted" in "waiting i/o"; which I confirmed looking at "top". So, in short: even while understanding that CPU would wait a little for i/o and that the system will be under a little load by writing a pen-drive, I find current situation beyond a normal experience. I am experiencing the same issue on the related Lenovo Y700-15ISK (same machine, 15" version) with Xubuntu 16.04. This seems to be an issue in grub, not related to the kernel. The error message string is from grub-core/disk/i386/pc/biosdisk.c in GRUB2. As far as I can understand the code, the BIOS returns some kind of failure code when GRUB attemps to write to the disk during the recordfail command. Deleting /boot/grub/grubenv works for one reboot, but it is recreated every boot. After some troubleshooting I have arrived at a workaround: edit /etc/grub.d/00_header and comment out the line "set have_grubenv=true". Disabling this line "tricks" grub into thinking grubenv is missing, so it never tries to write the recordfail data and no error message is displayed. I'm sure there are drawbacks to this, especially if the menu is skipped by default, but I always have a menu shown for dual-boot so I'm not too worried about that. I have no idea what's happening that causes the write to fail and don't know how to, for example, print the exact error code from the BIOS. The system cannot boot in recovery mode either. While fixing the specific bug in the uswsusp package might not be very urgent, the experience I had was that doing an Ubuntu upgrade to Xenial LTS edition caused my machine to be unbootable. If this issue is not going to be addressed then perhaps we should look into making the recovery mode more resilient to apparently unsupported scripts. Shouldn't the uswsusp script be disabled until the issue is resolved? The card seems like it is working as the modules are loading without an issue and everything is showing up in the sound settings as expected. Also alsamixer seems fine. The problem is that when ever I try to actually use the 2496 card the audio gets stuck. If I try to change to either analog or digital output of 2496 it will not make the change at all. For example VLC will try to play files but the media controls wont do anything. The VLC will not shutdown either. It has to be killed. Public bug reported: I have Audiophile 2496 and quite fresh install of Ubuntu 16.04. The card seems like it is working as the modules are loading without an issue and everything is showing up in the sound settings as expected. Also alsamixer seems fine. The problem is that when ever I try to actually use the 2496 card the audio gets stuck. If I try to change to either analog or digital output of 2496 it will not make the change at all. For example VLC will try to play files but the media controls wont do anything. The VLC will not shutdown either. It has to be killed. I've tried testing both those kernels, but couldn't boot wither either of them: I have full disk encryption set up on this laptop, and when booting these kernels I'm unable to enter my decryption password. The UI appears, but pressing keys on the keyboard does nothing - the UI doesn't update, and pressing 'enter' doesn't cause the typed password to be accepted either. The symptoms of the freeze are: * The two external monitors connected to the dock seem to be in a half-configured state - they're powered on, and display some graphics from my desktop, but do not yet show the correct contents. * I can't get to a console with Ctrl+Alt+F1. * I can't kill X with Ctrl+Alt+Backspace * I can't get the mouse cursor to appear or move, either with the trackpad or the external mouse. * Undocking the laptop doesn't help. The only thing I *can* do at this point is to hold down the power button. I seem to recall similar issues in the past (perhaps in Vivid?), where the fix ended up being in the graphics driver. The laptop is a T540p. The docking station is an "ultra dock pro" - not a USB dock, but rather one of those ones where the laoptop clicks into a base. I've just added "After=local-fs.target" to /lib/systemd/system/zfs- import-cache.service and now it starts almost normally. Only once I saw an error after boot saying that kernel module zfs is not loaded. I have an AMD A10-5800K (HD 7660D graphics) and have not been able to boot with 'nomodeset.' I get an error message, "ERROR: no UMS support in radeon module," and then in a few seconds the screen fades to black. I've had no problem booting from my USB with 14.04 LTS installed, or from the 15.10 LiveCD, but 16.04 seems to be hopeless. The system will not boot. Only a blank screen after grub loads the kernel. The recovery kernel loads fine and I was able to find the following messages: [drm] VGACON disable radeon kernel modesetting [drm:radeon_init [radeon]] *ERROR* no UMS support in radeon module! The recovery kernel command line had a nomodeset parameter that was not in the main kernel command line. Adding the nomodeset parameter the main kernel command line finally booted the system. It's kind of ugly but it seems to work. Note: For what it's worth I reported a similar issue against several Utopic kernels (bug #1407505). There was a back and forth with jsalisbury until a kernel finally worked. I have had no problems until today. Hope that helps. Behavior with 4.6.0-040600rc7-generic appears to be even saner than with 4.5.0-050400, although the AMD GPU is still not usable ("DRI_PRIME=1 glxgears" opens a black window). Only after reporting this bug, I found that 4.5.0-050400 presented issues with sound output in the HDMI port (sound output at a lower sampling rate, maybe half, than it was supposed to). These issues are fixed in 4.6.0-040600rc7-generic. This bug is still present for me with a fresh install of xenial. I have a dual-monitor setting with DVI and HDMI. As noted before, the behaviour is erratic, it does not happen every day or after each locked screen. Suspending by closing the laptop lid results in the black screen when re-opening. Once in black screen mode, the only recourse is power off re-boot. Although power manager is set correctly to "Suspend" in the power manager , on closing the lid the system does NOT appear to suspend as it does when the "Log-off Suspend" button is clicked or the hardware "Suspend" key is pressed i.e. the problem APPEARS to be on lid close. Power manager and light-locker set NOT to lock on suspend or shutdown makes no difference to behaviour. Xubuntu up to 12.04.4 (plus latest updates) worked absolutely correctly before new install of 14.04.1 This is a MAJOR inconvenience for an upgrade of an LTS.
