I could see this in the temporary folder and in "Recent Documents". In a "normal" Writer file this thumbnail will be saved inside the separate folder "Thumbnail" in the *.odt-file. Having a form inside a database there wouldn't be created such a folder and the thumbnail won't be saved. Let us save this thumbnail like it is done in Writer documents and show it in form preview.
