On Fileopen, the table and footnotes look good to me now.
I can see the table moved to page 2 in version 6.1, but that has improved in
recent version.

However, using the steps:

1. Open File
2. Insert footnote inside table

Or:

1. Open File
2. Tools > Footnotes/Endnotes settings > Footnotes > End of document > OK
3. Undo twice

... I can see the issues described: table starts on page 2, and much of it not
displayed.

1. Download and install oldest version of LibreOffice (usually 3.3 unless your
bug pertains to a feature added after 3.3) from
https://downloadarchive.documentfoundation.org/libreoffice/old/

2. Test your bug
3. Leave a comment with your results.
4a. If the bug was present with 3.3 - set version to 'inherited from OOo';
4b. If the bug was not present in 3.3 - add 'regression' to keyword
