What you see is just a space characters, scattered around the document.
They are not related to checkboxes.

After loading the PDF in Draw it is possible to select the objects that are
shown as checkboxes. While PDF readers show it as wingdings chars I can set the
line style from None to Continuous and see eventually the rectangular shape.
It's the same for bullets where also white shapes are inserted as placeholders,
or for whatever reason.

Checkboxes are not the shapes.
They are characters of embedded Wingdings font.
This is definitely the same problem as Bug 63834.

Still valid (5.2 alpha). The checkboxes are there as shapes but with line style
'none', meaning hidden. No idea where the check mark has gone. 

Could be related to 63834.

I can't find a version in the bibisect 43all repository which correctly imports
the checkboxes. (3.6.5.2 release on OSX also doesn't show them)

If this has ever worked it's somehow only done so on Windows.

I can confirm this behavior using your attached pdf; tested with Linux Mint 14
x64 and Libreoffice 4.0.1.1 rc and Version 4.1.0.0.alpha0+ (Build ID:
