Can't \includegraphic in preamble or ERT

I have the layout of a Memoir book title page defined in LyX using  
LaTeX, because it is too complex to be done with the LyX editor.  One  
line of it is this:


\includegraphics[scale=0.1]{terminuspress.png}

terminuspress.png is in the same directory as the LyX file, but I've  
also tried using a full path to terminuspress.png.  In either case, it  
comes out on the title page as an empty box - the correct size, so  
obviously it is seeing the image at one point - and the full path to  
the image printed across it.


It makes no difference whether I have this line in my preamble, or in  
ERT in the body of my document, or even - maddeningly - if I break the  
ERT and include the graphic in normal LyX mode then resume the ERT.   
However if I put the line in ERT in an otherwise *blank* document it  
does work...?  Also, I do have other graphics in my text that come out  
fine.


Does anyone have any suggestions as to why I get the empty box?

Also, a completely unrelated question but not worth making a separate  
message about, in my table of contents I have long page numbers like  
xxxiii that go over the right margin to which all the other page  
numbers are justified.  Can anyone suggest how to get them to line up?

Some of you may be aware of the nice Python scripts called ConvTex which 
Nick Thomas wrote to convert from LyX (via LaTeX) to OpenOffice formats. 
 (If not, see http://wiki.lyx.org/Tools/LyX2OpenOffice.)


Well, I've just written a Mac front end, ConvTex.app, which is linked 
from the URL above.  It is easy to use.  Simply open the application and 
select the file to convert, or drag and drop one or more files onto the 
application's icon.  They will be saved to the same spot as the original 
files.


Since Nick does not have a Mac, I would be grateful if some of the 
people on this list who do would test it out and let me know how you get 
on. Please feel free to let other people know about ConvTex.app too.

might have missed one.


No, I couldn't see any either - looks like they have been removed.  This 
is a show-stopper for me unfortunately so I've had to downgrade back to 
LyX 1.3.6 for the foreseeable future.  Thanks anyway.

I have just upgraded from LyX 1.3.6 to 1.4.4, and I can't insert 
references using the minibuffer or LyX server anymore.  I used to use 
something like:


  reference-insert ref|++|label

but now LyX tells me Unknown action.  Similar commands like 
math-insert still work.  Has the command syntax changed?  I couldn't see 
anything documented.

references using the minibuffer or LyX server anymore.  I used to use 
something like:

but now LyX tells me Unknown action.  Similar commands like
