Track down your preferences file (should be something like "C:\Documents 
and Settings\\Application Data\LyX1.4.x\preferences". 
Open it with a text editor (e.g. notepad), find the line


\ui_file "stdtoolbar"

and change it to

\ui_file "default"

then save and start LyX.

You should remove the Lyx Folder in the application data folder.

Got to c:/Documents and Settings/Your User Name/Application
Data/Lyx1.4.x

Delete this folder and it should work. I am not sure whether there is
any other way though
