Another alternative is something called doublecommand -
it's a kernel extension that is managed through the preferences panel.
It's open source, and it's available here:

Speaking of voice support for Firefox - there is an extension to
Firefox that supports something like VoiceOver.  The extension
is called FireVox, and it is available here:
http://firevox.clcworld.net/

When we took a look at it, the deal breaker for us was that
it could not switch voices on the fly, and we needed to be able
to switch from English to Spanish voices, without logging out
and back in...

I don't know if this will work, as this RNIB streaming book service
says it is only available within the UK - but I found a DAISY
reader application for MacOSX, called Olearia, here:

It is still a beta product...

As I recall, we did not try to combine VO with Firefox (and the
FireVox extension).  IIRC, FireVox on its own was able to
read content shown on the Firefox window aloud, which was
what we were looking for at the time.  I don't remember whether
we ran into any other issues when trying it out, though - but
I will try to install it again, and play with it a bit, then report
back.
