Hi,

Rogue Amoeba sells a software package called SoundSource. It allows you to 
individually set audio for just about any device that you can imagine. It is 
very VoiceOver accessible.

Yeah, I came across Audio Hijack, but it's for recording to file.
Useful, but a different use case.

Once upon a time there was a port of the Linux jack application, but it
seems it's unmaintained. Even it's old web page,
http://www.jackosx.com/, has expired and no longer resolves to anything.

Jack does serve this function on Linux. You can make the output of one
application go to whatever device or other application you want to
specify. Basically, it's a software patch bay.

Seems I was just dreaming!

Yes, I see where all audio can be directed to a different device, and I
even see how these devices may be aggregated.

However, it's also theoretically possible to direct audio output from
one application to one device, while directing output from a second
application to a different device. This doesn't seem to be supported by
the Apple OS, only by individual applications that care to define that
functionality.

Oh well, a girl can dream!

Janina,

Not sure exactly what you are wanting to do, but you can control the default 
sound output in the Sound preferences in System Preferences.
You can control VoiceOver output from within the Sound category of the 
VoiceOver Utility (accessed by VO + F8)
