Hi!
Go into VoiceOver utilities with vo f8 and go to the files menu.
Arrow down until something like  reset VoiceOver.

But can you maybe reset voiceOver to see if that helps?
Do a backup of your settings first.

I would suggest contacting Apple and creating a support case. Other 
people have not been reporting similar issues recently either here or on 
the AppleVis forum. It's likely specific to your situation.

Voiceover frequently shutting off

 I am running the latest Sonoma on a MacBook Pro M3. Lately, I find voiceover 
needing to be rebooted because it stops speaking. I don’t know if it has been 
shut off or somehow muted itself but I can get no speech from any action. So, 
the solution has been to reboot voiceover with the command F5. I first hit 
command F5 to shut it off, and then command five again and voice comes back up 
speaking. i’m worrying about the time when it won’t come back up speaking. This 
is an oddity I don’t understand. Why is voice needing to be rebooted on a 
regular basis?
