Why do you change your underpants? A few skid marks are not a problem, right? The problem with a dirty engine is that some of the crud may block/partially block an oil passage, leading to problems. Also a cleanish engine will stay cooler because oil is also a cooling agent. End of the day they are not designed to have half a tonne of sludge sitting in the engine and if it is possible to clean/keep clean it is advised to do so. If it was just in the bottom of the pan it would not be such a drama but when the top end is sitting under half an inch of black goo it is a worry.
