I just tried with a fresh OpenSMTPD setup and it worked fine. I don’t know why 
it didn’t before. It was a few years ago that I last tried. Sorry for the noise 
- and thank you for OpenSMTPD!

Maybe it is a baremetal versus VPS thing? Like others here, I have been
running a mail server on Vultr VPS for years without problems.

The problem is that you have only configured virtual users for external 
connections.

When roundcube (which you say uses localhost) or your users (authenticating on 
port 587) send an email, they hit this rule:

accept from local for local alias   ...

That uses the  table, which evidently does not contain your virtual 
users.

When external (not from local) connections are made, they hit this rule:

accept from any for domain  virtual  ...

That uses the  table, which I guess contains all your virtual users, 
which is why it works.
