This is bizarre. Some data:
1. The probability of this occurring increases, if the CPU is very busy, at
about 10%, i.e. running 'while true;  do true;  done' in parallel. It also
increases when opening files I haven't opened before.
2. Font generation seems to be irrelevant: I deleted the cache of pk files
(/var/cache/fonts) to no avail; they aren't recreated and dvi files display
fine. Note: I have never seen the generating font message I remember from
KDE3 times.
3. The dvi file is _not_ opened, while okular is hung, according to lsof.
4. I can reproduce this with:
\documentclass{article}

\begin{document}
testing
\end{document}
in a tex file.

I was viewing a dvi file that I just updated by running LaTeX on a tex file.
Okular stalled in updating the file and after I quit okular I get the crash
message.

When you select, copy and paste a paragraph of text with okular's selection
tool then hyphenation is not handled correctly.
I.e. if a PDF contains this paragraph:

We, therefore, the represen-

of America...

and you copy and paste it into another application via okular's selection tool,
then the text *should* read:

We, therefore, the representatives
of the United States
of America...

This is also how Adobe Acrobat behaves.

I found similar bug, but couldn't reopen it.

I installed KDE 4.4.0 on Windows XP x64 SP2, using MSVC 32bit packages.

In temporary folder there is file:
poppler-vc90-0.12.2-bin.tar.bz2

According to

I downloaded and extracted this file
http://winkde.org/pub/kde/ports/win32/releases/stable/latest/openjpeg-1.3-1-bin.tar.bz2
but this lead to crash of okular after opening any PDF file (unhandled win32
exeption), debugger shows access voilation in malloc.c).

I suppose it is installation bug, and I have manually downloaded incorrect
package.

Version:   0.9 (using 4.3.00 (KDE 4.3.0), MS Visual Studio 2008 SP1)
Compiler:  cl.exe
OS:Microsoft Windows (unknown) release 6.1

Okular doesn't open PDF files in the KDE 4.3.1 and KDE 4.3.2 builds for
Microsoft Windows, when trying to open them on Windows 7 (Release Candidate
build 7100). Other files are fine (at least the JPGs I tested).

I uninstalled all the files, then downgraded to KDE 4.3.0, and now it works
fine.

I'd say the culprit is poppler 0.12.0, shipped in since 4.3.1 release. In the
0.12.1 release notes they say they fixed a lot of WIN32 issues, which might be
related to this problem.

shown version is taken from other available pixmaps for that page (for example
the thumbnails).

Not really; did you have the preview active in the file saving dialog? If so,

Sorry, I was in a hurry when I filed the bug, but yet would not want to
miss the opportunity to inform the bug tracker of a problem now that
it's so easy with the automatic bug tracking mechanism in KDE —
particularly as it was the first one I brought through (I had to
compile a lot with -ggdb and splitdebug on to generate useful stack
traces) ... and on top of that: without some routine you just don't know
what really is useful information to pass along.

I tried to reproduce the failure with and without preview on: 

The problem does not occur systematically, not even with preview on;
which leads me to suspect that it has to do with concurrency (maybe
a race condition), and that it may happen if I close Okular while it is
still saving the PDF copy... the files that crashed (3 in number) were
all downloaded VISA account reports — that might be another reason
(something foul in the PS-code? Still, the document showed up
flawlessly, and Okular only crashed immediately after closing the
frame).

I have preview off now and will add to the bug report if crashes still
occur.

Don't hesitate to ask more questions to pinpoint the problem.
