Congratulations Carolyn! It takes a lot of guts to really take a stand. One quick thing - I was so impressed with the Democrats policy on voluntary organisations and work. Amongst the highlights include a scheme whereby HECS debts can be paid by doing voluntary work. As I know the personal paybacks of voluntary work I highly commend the social building this type of initiative encourages. Let's all go to the polling booths with a strong clear intent for change in birth as a result of this election. Hold the dream for us and for our daughters. Unedited version follows In Birthing Solidarity Justine Caines Maternity Coalition - ACT Branch __ Once again Obstetricians have threatened not to practice in response to skyrocketing professional indemnity insurance premiums and large compensation claims for birth trauma. This elicited a prompt response from Bob Carr. For some months Independent midwives have faced a similar crisis. The standard response to midwives request for help is no as that this will set a precedent and then other practitioners will want help. Midwives and Obstetricians are both health professionals that provide assistance and support to pregnant and birthing women, a like service. Obstetricians are specialists who should only provide services to the minority of women who need medical care. The World Health Organisation suggests up to 85% of women should be cared for by a midwife only. Over the last 10 years there have been at least 6 state and federal inquiries into maternity services with little if any recommendations acted upon. In New Zealand women are able to choose their carer and now 72% choose a midwife only. Australian women, however, are forced into costly, unnecessary medicalised care. Less than 5% of Australian women can access midwife-led care, deemed to be the most appropriate and cost effective care for the vast majority of women. Many current obstetric practices are not based on evidence but on hospital schedules and doctors fear. Perhaps this is why the latest maternal deaths report shows an increase of direct maternal deaths (those that were preventable) increasing from 27 to 46 until 1996. Dear Bronni It's great to hear of your meeting in Healesville. I'll have a go at answering your questions. Although I am a member of the Board (I think I'm the only one on this list), I am also finding the whole PI insurance issue very difficult. The Victorian Nurses Act 1993 has recently been amended, and the amendments have come into effect 1 Nov. The new section 5 (3) (d) states: [The Board] may require the applicant [for registration] to provide evidence that the applicant is or will be covered by professional indemnity insurance that meets the minimum terms and conditions set out in guidelines of the Board. Does this mean the Board can deregister someone (ie an independent midwife) who is not insured? It probably does. I have been assured that it's a DISCRETIONARY power - but even that doesn't make me feel very comfortable. It will probably begin to take effect as people apply for endorsement as Nurse Practitioners, if they want to work outside the hospital system. (for those in other places, the NP endorsement will enable a person to have extended practices - prescribe, order tests, refer to specialists.) I believe we need to get insurance. ACMI members would have received a discussion paper and ballot to return, so that we can choose option 1 or option 2. I and other independent midwives look forward to resolution of that issue, as it's the only insurance on offer at the moment. I hope AHMAC (health ministers advisory c'tee) under whatever new government gets up on Saturday will set up a system of no-fault insurance similar to what's done in NZ. I see no future with spiralling costs for insurance. Doctors who belong to the Medical Defence Association of Victoria (an insurer) have already been told: Warning to members in obstetric shared care. Members should be aware of the recent decision of Guild Insurance to withdraw the provision of indemnity to midwives. This may spell the end to midwife-supervised home deliveries as a choice for the community. The statement goes on to warn members against working with uninsured midwives! (Medical Defence Update newsletter, Winter 2001) Consumers and midwives need to consider what this means, and keep on working together to resolve the PI crisis. "Caesarean section is a surgical intervention it's major surgery and one doesn't want to have surgery unless that is a safer option for someone than not having it," said Professor Lesley Barclay of the Centre for Family Health and Midwifery at the University of Technology, Sydney.The researchers say they don't know whether the increase in Caesarean deliveries is responsible for the increase in deaths, or whether it's because women are waiting longer to become pregnant.Professor Barclay, a member of the NHMRC's Advisory Committee on Maternal Mortality and Morbidity, said that whatever the cause, some of the deaths were avoidable."For some of those cases, appropriate standards of care were not met, so that is serious."Overall, Australia's maternal death rate remains low by world standards, at one in 7,675 births. Hi Jackie Independent midwifery is alive and well in Sydney. There is only around 10 midwives in Sydney currently practicing. A number of midwives have stopped practicing due to the withdrawal of professional indemnity insurance. Have emailed Leilah directly with the information she requested. The Australian Society of Independent Midwives is active in Sydney as is Homebirth Access Sydney (homebirth support group).
