Re: [ozmidwifery] no subject

2003-02-13 Thread Marilyn Kleidon
I have noticed that mums are even afraid to put their babies on their
tummies to tie up their nightie. I assure them it is ok and even beneficial
for babies to spend time on their tummies, just that you need to be
observing them. Of course I slept all of my girls on their tummies, but I
was picky about having clear breathing space, I also used a NZ lambskin for
all 3, but pulled a sheet tight across the area under their face. So, I am
not surprised that babies may not spending enough time on their tummies to
use their neck muscles, though I think it is a good idea that they sleep on
their babcks, and am increasingly surprised at how easy it is to do this.

 Dear friends,

I don't know if this topic has been discussed in the group already, but it
interests me. The sleeping rules for babys (not on their tummy anymore)
causes a lot of problems with back and neck muscles of children 3-4 months
of age and later, because the parents are made so anxious that they even
in
daytime don't put their baby on the tummy anymore.
On the website of midwifery today is a study (in Englisch :-))

It's an New zealand and UK study. In our country it is unknown. Do you
all
know more or have some toughts about it?

This is my gut reaction and maybe inappropriate

the baby will stay breech and this is not good,
The baby can turn or be turned at different times
until labour is well established - only God if and when (and maybe the baby)
  knows ??
But come back to seeing the
  positives.
As I read the history of birthing- the fear

Hi all,

I'm writing for ideas for a friend, has had a previous
LSCS for failure to progress after an induction, now 39 weeks and has just
found out baby is breech. Desperately wants to try for a VBAC, now
thinking it's all out the window. What I need from you wise women, is
a shortlist of what I should suggest to her, as she doesn't have much time,
and going in for accupuncture treatments, or trying hands and knees might
just not work in time. Is it reasonable to suggest she try an ECV at
this late stage ( I know there are increased risks associated with a
previous LSCS, but she may decide that those risks are fewer than those of a
repeat section). I want to suggest to her the things which have the
best chance of working, while being honest about the risks, and failure
rate, so she can make up her own mind.

Thanks

Tania

Re: [ozmidwifery] re: breech presentation help

2003-02-13 Thread Lynne Staff

I know - when I was a student midwife, I worked
with an older ob who used to say "Sis, Hands off the breech until the mother has
brought the baby down to its armpits - then, if assistance is required, do
so."

I was fortunate to "grow up" with this way of
knowing breech birth - it is fast becoming extinct. The fact of matter is that
breech birth is most probably more dangerous now than it has ever been, because
the "Professionals" have never had the experience of breech birth apart from
caesarean or (ugh) Breech Extraction (a forcible birth, and not a pretty sight,
often with fetal and maternal injury).

As for physiological
breechwell!

"Wouldn't it be nice if she could just be
supported to give birth with the baby breech? I read with sadness the
responses all suggesting ECV, and thought this is a sure sign of the times.
Big sigh."

Lynn,

My thoughts exactly, but the reality is that she will not
be supported in birthing a breech baby, and may not be supported by her Ob
if she decides to have an ECV, which will have to be in the public system,
with another Ob. What a fragmented and crazy system it is, when a
woman fears retribution from her maternity care provider for seeking the
help of a specialist who may give her the chance to have the birth she has
worked so hard to mentally and physically prepare for. Even bigger
Sigh...

[ozmidwifery] Birth outcomes

2003-02-13 Thread Andrea Robertson
I've been reading the NSW Mothers and Babies Report (2001) with great
interest over the past couple of weeks. The figures tell a shocking story -
of failed opportunity in midwifery led units to gross overservicing in the
private sector. I've explored some of the outcomes in My Diary entries over
the past week

I hope that the new Royal Prince Alfred Hospital Womens and Babies Unit can
improve its figures - - they are not as bad as some, but fall a long way
short of good. After my experience of their approach to public relations,
support during labour, attitudes to normal birth and silly water birth
policies I wrote to several people suggesting that a meeting to discuss
improvements might be in order. Sadly, they don't seem to think that it is
important to follow up on letters, let alone discuss the issues that I
raised - I have had no response of any kind. They did phone the mother
concerned, catching her at a time of stress when she was still in that
early days haze. She corroborated my comments in the letter (which she
hadn't seen) but all they seemed concerned about was obtaining an assurance
that she would use the hospital again for her next birth. I think she will
probably go elsewhere (she's even thinking now about home birth).

Such a contrastt to the staff at St George Hospital where I presented an
Active Birth workshop for their staff and students last week. Everyone,
from Jo Wills (the Patient Services Coordinator) down, was positive,
enthusiastic and committed to woman centred care. Their outcomes could be
better as well, but there is a strong feeling of positive action being
taken and a united philosophical approach driving their plans and care
patterns.

Andrea

[ozmidwifery] RPA replies!

2003-02-13 Thread Andrea Robertson
I have now had a response from RPA regarding their waterbirth facilities
and my letter of complaint. They have ignored the request for a meeting
with me, but assure me that 36 of the 40 midwives have now been accredited
which includes lectures and completion of a workbook, on all indications
for use of water in labour, the infection control policy, the manual
handling policy and issues pertaining to same, and the occupational health
and safety requirements.

This is good and is a useful first step. The other issues I raised of staff
skills and attitudes around woman-centred care, inflexibility in the
application of rigid admission procedures and poor public relations with
support people have not been addressed at all. They still have a long way
to go. but at least their staff will be able to look after their own
needs if they offer a woman the use of the bath during labour.

(Sigh) When will hospitals realise that listening to women shoud be the
first step in the process of supporting normal birth, not a bothersome
annoyance they have to deal with (but only when pressed)? I offered some
insights into the way their service is perceived by outsiders, but this
letter does nothing to assure me that they have taken any action on my
obervations. They'll be putting this matter to one side now, having felt
they have dealt with me. Sadly that is not necessarily the case. Public
relations involves more than just trying to shoot the messenger and
vague references Your comments have been noted and discussed at both ward
and departmental level. It is not too late to do something about this -
and I will hopefully have a chance to see how things are going in the
coming months when I will probably be once again be supporting women who
have chosen to go to RPA.

RE: [ozmidwifery] strep B

2003-02-13 Thread Jane Palmer

Dear
Trudie

There
is a website in the UK on Group B strep - it has leaflets as.pdf files
that you are able to download - Check it out at http://www.gbss.org.uk/. I
providewomen with one of the leaflets off this site and talk about the
variations in policies at different hospitals. Some women will choose to have a
swab in pregnancy (generally late pregnancy) - if positive I suggest a
naturopathic/homeopathic consultation. I discuss IV antibiotics in labour at
home and their feelings around the same. On a couple of occasions women have had
IV antibiotics at home - heavy growth of GBS remained after they had taken
natural remedies. If a woman has SROM and no labour - a recommendation is made
to have a low vaginal swab as soon as practical (given it takes 48 hours for the
final result) - so her GBS status is known.

Hope
this helps

Well my friend has had one treatment of moxibustion, and last
night I saw her, and her very mobile and active baby. Her baby is
currently lying in a transverse position, happy as anything, and has been head
down for the most part of her pregnancy, partially engaged only a few weeks ago,
so hopefully is on his/her way to getting down there again. She's going to
continue with the moxi trieatment, going again early next week, seeing a chiro
today who is well versed in the Webster technique, and will continue with doing
lots of position and posture work herself. I printed out your replies for
her, and she was overwhelmed with the kindness and amount of knowledge you were
all ready to share for a stranger.

Thanks one and all,

PS Is there anything I've missed (always second

Re: [ozmidwifery] re: breech presentation help

2003-02-13 Thread Marilyn Kleidon

Tania, it is my understanding that the moxa should
be done daily, at home with the moxa sticks until the baby turns, it is
definetly not a once/week thing.

Well my friend has had one treatment of moxibustion, and
last night I saw her, and her very mobile and active baby. Her baby is
currently lying in a transverse position, happy as anything, and has been head
down for the most part of her pregnancy, partially engaged only a few weeks
ago, so hopefully is on his/her way to getting down there again. She's
going to continue with the moxi trieatment, going again early next week,
seeing a chiro today who is well versed in the Webster technique, and will
continue with doing lots of position and posture work herself. I printed
out your replies for her, and she was overwhelmed with the kindness and amount
of knowledge you were all ready to share for a stranger.

  Thanks one and all,

  Tania

PS Is there anything I've missed (always second
  guessing myself!)

I agree that many women are afraid about placing their babies on
theirtummies - I think there is so much literature around about the
SIDSrecommended position to put babies to sleep on their back that they
haveto be reminded to allow "tummy time" during wake times. Also I
heardthat some children are ultimately suffering from misshapen heads,
onewoman was telling me that her son had to have a special helmet made
tocorrect the shape of his head. Consequently when she had her
secondchild she ignored the SIDS recommendations and placed her baby from
sideto side to sleep. Janet -Original Message-From:

Re: [ozmidwifery] no subjectI have noticed that mums are even
afraid to put their babies on theirtummies to tie up their nightie. I assure
them it is ok and evenbeneficialfor babies to spend time on their
tummies, just that you need to beobserving them. Of course I slept all of my
girls on their tummies, butIwas picky about having clear breathing
space, I also used a NZ lambskinforall 3, but pulled a sheet tight
across the area under their face. So, Iamnot surprised that babies may
not spending enough time on their tummiestouse their neck muscles,
though I think it is a good idea that they sleepontheir babcks, and am
increasingly surprised at how easy it is to dothis.marilyn-

received this a few weeks ago. sent it
to my brother who's diabetic. he sent it on to a friend of his, an eminent

Thanks all for the reply, but I didnt make my question clear.
I read the following study and I was wondering if you had heard about it.

PhD (http://www.midwiferytoday.com/articles/bedding.asp

When this is true, babys can sleep in there favorite position all times if the matras is wrapped and we dont have to make parents anxious anymore.
