[ozmidwifery] Maternity care forum in Adelaide Hills...

Hello everyone,

Firstly, please forgive me if you receive this mail more than 
once, there are several of you who are on more than one list and I wanted to get 
this out quickly, so no time to fuss around with checking double 
posts.

Now, onto the business side of this VERY IMPORTANT MAIL. 
There is a forum happening at Mt Barker, to do with the future of maternity 
services here in the Hills, and although not all of you live up here, it's 
really important that we get a huge number of women to attend. They need 
to hear that there is demand for one-to-one midwifery services here in the 
Hills, and all over Adelaide too!! They need to hear that women want to 
have their own midwife, and that they wish to choose where they birth, sometimes 
making that decision before they are even pregnant, and sometimes when they are 
in labour, including the option of homebirth. They need to hear that women 
want to have access to a hospital when it is needed, and that they want to be 
able to bring their own, professional midwife in with them, to give them care, 
and follow them through until after the birth of their baby. Get the 
picture???!! At the moment, women in the Hills, and now in the West of 
Adelaide with the (temorary) closure of the QEH birthing services, don't even 
have access to a Birthing Centre, let alone anything that represents real 
woman-centred continuity of midwifery care.We won't get it 
ifwe don't let someone know that we want it,so here is your 
opportunity to let them know! Come along and support the midwives who are 
working towards change, and the women who want and deserve it. And if you 
don't want to say anything,that's ok,what we need is a huge amount 
of women to attend,in a show of banding together to support each other, 
and to let them know that we want something better than what is available at 
present. Just being there in the numbers will be a valuable contribution 
to helping make a difference. Let's pack that hall to the door, and let 
them know that we do have an opinion, and that we do want something more than 
what's on offer.

Hope this has motivated lots of you to come along, this might 
sound like a littleHills thing, but it might also be the start of 
something big for women all over Adelaide.Please pass this mail onto 
anyone you think might be interested in coming along in support.

[ozmidwifery] question

hi everyone,
my name's bec, I'm a student midwife. One of my friends is pregnant and she has 
what I think is called restless leg syndrome. She says it feels like ants are crawling 
over her legs all the time and it's driving her crazy! I have not really come across 
this 
too much and I haven't heard of any ideas of what may help relieve this for her. I 
think 
her midwife suggested maternity stockings may help, any more ideas anyone?

that endorphin release is higher after acupuncture and TENS. Is acupuncture used 
in activelabour? I would be interested to hear of anyone's experiences of

Hi, 

I used the sea bands with my 3rd  4th 
pregnancies  think they are wonderful. By the time I was pregnant 
with the fourth I couldn't stand the smell, taste, or thought of ginger... I did 
use the tablets though to try  get me over the worst of it.

the Journal of Midwifery and Women's Health)  Volume 39, No.2 (Supplement), 
March/April 1994, pp.133 - 148   Nurse-Midwifery Management of 
iron-deficiency anaemia during pregnancy  by Janet L.Engstrom, CNM, Ph.D 
and Claudia P. Sittler, CNM, MS which discusses these issues and is 
excellent reading. It gives you an  algorithm for a differential diagnosis 
and pathophysiology of the various anemias and the beneficial and normal 
haemodilution of the healthy second trimester (no pathophysiology here). I 
have a photocopy of this article and could send copies by snail mail to 
anyone who wants, though I am sure you can find this journal in  some Uni 
library.

  I have just finished reading Michel O'Dent's book 'The Casarean' ( 
available from Birth International) which was very interesting reading. In 
it he discusses measuring Hb during pregnancy and I will type the relevent 
section from pages 115-116

  He states that
  '... there is a widespread belief that this test can effectively detect 
anaemia and iron deficiency. In fact this test cannot diagnose iron 
deficiency because the blood volume of pregnant women is supposed to 
increase dramatically, so the haemoglobin concentration indicates first the 
degree of blood dilution, an effect of placental activity. A large british 
study, involving more than 150,000 pregnancies found the highest average 
birth weight was in the group of women who had a haemaglobin concentration 
between 8.5 and 9.5. Furthermore when the haemaglobin concentration fails 
to fall below 10.5 there is an increased risk of low birth weight, pre term 
birth and pre-eclampsia. The regrettable consequence of routine evaluation 
of haemoglobin is that all over the world, millions of pregnant women are 
wrongly told that they are anaemic and are given iron supplements. there is 
a tendency both to overlook the side effects of iron (constipation, 
diarhhoea, heartburn etc) and to forget that iron inhibits the absorption 
of such an important growth factor as zinc. Furthermore, iron is an 
oxidative substance that can exacerbate the production of free radicals and 
might even increase the risk of pre-eclampsia. '

  Well I knew the first bit about blood dilution but was not aware of the 
study that showed the higher birth weight amongst the women with lower Hb 
or the tendancy to pre eclampsia to women with higher Hb. Makes sense 
though that if there is higher blood pressure then placental function/flow 
will be decreased and lead to lower foetal growth. Makes the whole thing a 
bit of a joke though doesnt it as Hb is the one thing I thought was worth 
knowing of the battery of tests that women are subjected to. Maybe we 
should be celebrating when there Hb comes back lower that 10.5 and 
'worrying' when it doesn't. Cant you see it now new indication for keeping 
an eye on BP Hb above 10.5'. Food for thought though. As it will make any 
though of supplements virtually obsolete.

[ozmidwifery] Djerriwarrh festival (Melbourne)

Maternity Coalition (Vic) is holding a BBQ/pamphlet
table at the Djerriwarrh (pronounced JERRY-WARRAH)
festival in Caroline Springs (in Melbourne's Western
suburbs) on November 7th.

This will be a great opportunity to work in
partnership with a rapidly growing community.  Our two
primary objectives for the day are to 1. Raise much
needed funds for MC, and 2. Promote and raise
awareness of midwifery led care in the community.

It would be great to have consumers  midwives (
students) represented to help out!

The hours we need volunteers are 9am - 5pm on Sunday
7th November.  We are asking that people come for an
hour or two hour block.  Maybe people might want to
come for an hour or two, have a look at the festival
and come back...it should be a great family day with
lots of rides, jumping castles, craft displays and
games for the kids.  All of the organisations are
community based, so prices should not be too inflated!

[ozmidwifery] Accupuncture

I had an accupuncturist give me accupunture 
in the days leading up to my birth, the night before I went into labour, during 
active labour in her clinicand then I asked her to come in to the birth 
centre with me, and she did give me some accupunture during pushing 
stage.I think it helped get things moving as I was overdue and running out 
ofdays to be able to go to the birth centre, I don't know to what degree 
it helped with the pain,I had a wonderful birth with nopain 
medicationand I wouldn't describe it as painful either, it was hard work 
and a long second stage with a nine pound five ounce baby exactly one year ago 
today. Iwould use accupuncture again.I don't know if that helps, but 
if you have more specific questions I'm happy to answer them.

[ozmidwifery] RE: acupuncture in active labour

Hi Fiona , I have seen one woman bring her
acupuncturist into hosp with her. He used points to assist her in first
stage, then left her to it. She laboured really well and the needles didnt
appear to bother her too much or restrict her movement. If my memory
serves me right it was her first baby and she had a labour all up of about 4
hours. I think the points he used were more for helping coordinate her
irregular contractions rather than pain relief, but she didnt need
anything else. From a midwifes point of view it was very successful-
beats syntocinon and monitoring! Sharon
