You can:-))

Join MC Œcause we make representations like this all the time.  In fact I
will raising this issue with the Shadow Hlth Mins in a couple of weeks.
Write letters to your local/state papers asking why the total lack of
competition and care that is not based on sound health outcomes etc etc

Inform all your CBE clients that PHI (Priv Hlth Ins) is totally
discriminatory and does not allow choice (ie no midwifery care).  It will
fund joggers and aromatheraphy and yet the most important decision for a
couple they are forced to use a priv obs at a priv hosp and by that very
fact significantly increase their chance of unnecessary surgery and
resultant morbidity.

Support anyone who has PHI who wants a mid choice to approach their fund and
request parity for maternity care (ie same costs covered for M/w¹s as Obs).
When as they no doubt will say no.  Take it further to the Private Health
Insurance Ombudsman

Points to make (which I know you already know!!)

*There are more babies born each year than any other health procedure.

*In private health insurance there is absolutely no competition, it is
totally medically dominated.

*It is also an area that is crippling health funds (so much so that smaller
funds actually tell you which Obstetrician you can see as they can only
afford some).  

*Some  funds are looking at providing a midwifery benefit (as they recognise
the huge cost saving) but where is the Government support??  Still no
assistance with insurance.

With insurance all funds could offer choice (that would also double as a
HUGE cost saver).  Importantly it would also make PHI a viable option for
rural consumers (currently as most rural areas do not have private Obs or
even Obs there is no point joining up).  This shows what a giant waste of
money PHI is in its current form.

capable of practicing without the supervision of a doctor in Australia. 
Therefore it will not recompense any services provided by them to pregnant 
women.

HOWEVER: The exception is when working in a remote community :
in which case you don't even have to be a midwife to provide pre-natal 
care, enrolled nurses  aboriginal health workers with NO mid education are 
considered quite competent to provide maternity care which medicare will 
refund for as long as they are supervised (however loosely) by a doctor 
(who of course knows much

more about normal pregnancy  childbirth then any midwife) !

If Midwives were as qualified as a chiropractor, chiropodist, naturopath, 
herbalist etc then they would get recognition.

It would appear that we just aren't up to scratch !!!
Our skills whilst working within a hospital immediately disappear once we 
step off the premises apparently.

I can add to this outrage.  I am a naturopath and as such could not assist 
any of my pregnant patients to birth their babies.  I had to go the 
circuitous route and study first as an RN  then 1 year postgrad to become a 
midwife.  Now, after this four years, as a midwife I cannot claim rebate 
from the system that trained me, but as a naturopath I can.  How bizzare 
is that?

There must be quite some variation between health funds then.  My HF
covers confinement and delivery at home as long as the care is provided by
a registered midwife in private practice.  I might actually call them
today and find out if they have a list of midwives who they deal with.

NO there is no variation,  this discussion is centred around MEDICARE...the 
Govt refund for fees charged. All women should be entitled to midwifery care  
to be recompensed for an agreed scheduled fee.
There is no recognition of a registered midwife in IP being qualified to 
provide the specialised care for maternity clients for which she has been 
trained.

Some private health funds recomp up to $2000 of a MIPP fee, it's up to the 
consumer to shop around for the best deal here as Justine pointed out in a 
previous email.

RE: [ozmidwifery] Breech Ob in S.A.

I’m not recommending anyone per say, because as we all know, the difference
between the philosophies of midwives and private ob’s are often
great…however, it is worth looking at Chris Wilkinson and Brian Peat at the
Women’s and Childrens, and I know Basil Antonas does vaginal breeches for a
second baby…not sure of any others that would put themselves out there with
that kind of stuff, there are a couple of  Ob’s at Burnside who recommend
women to hire birthing pools (there are no baths there in their labour ward)
– Karen Chandler and Tracey Bradbury, but I’ve never met either of them.
They are both of the opinion that women can ‘wallow’ in labour, but not
birth in water…not sure what the midwives that work there think about that.

 

Not sure if that’s helpful, but a few names are better than none I
suppose…something to go on with and start researching…of course, if she
wants something better than that she should be looking for a practitioner
who regularly attends waterbirths, and is comfortable with her birthing any
which way she pleases – perhaps a private midwife??  Many are very
comfortable with the idea of an uncomplicated breech, waterbirth, standing,
whatever… :-)

I think your friend would probably be safer with an elective c/s than 
attempting a breech birth in the private system. it is unlikely to be 
physiological but will be medically managed which is just not safe.

If she is seriously considering a breech birth it's important that the 
practitioner is confident in breech birth and physiological labour.  In both 
the public and private system the care is undertaken by midwives and the breech 
extracted by an Ob, this is not something I would recommend.

  My friend is hoping to get into the Midwifery Group Practice at the Womens 
and Children's in Adelaide. Places are limited though and rather than the 
public system being the second option she would prefer private care with an Ob.

  Ultimately her birthing preference would be to have a waterbirth... are there 
any Obs that are open to accommodating this request? At the same time... are 
their any Obs in Adelaide that will deliver a breech baby vaginally (says 
something positive for their philosophy on birth)?

  Any recommendation/help with this would be greatly appreciated!!
