Medicare does not recognise Midwives as 'professionals' competent or 
capable of practicing without the supervision of a doctor in Australia. 
Therefore it will not recompense any services provided by them to pregnant 
women.

HOWEVER: The exception is when working in a remote community :
in which case you don't even have to be a midwife to provide pre-natal 
care, enrolled nurses  aboriginal health workers with NO mid education are 
considered quite competent to provide maternity care which medicare will 
refund for as long as they are supervised (however loosely) by a doctor 
(who of course knows much

more about normal pregnancy  childbirth then any midwife) !

If Midwives were as qualified as a chiropractor, chiropodist, naturopath, 
herbalist etc then they would get recognition.

It would appear that we just aren't up to scratch !!!
Our skills whilst working within a hospital immediately disappear once we 
step off the premises apparently.

Usually means with a little more pressure on it from the bub's presenting 
part it should dilate rather easily cause it's soft and stretchy - a nice 
cervix

the cervix is 6cm dilated but can be stretched to 8cm...

Looking for a midwife in Araratsee post below Can anyone help?

Hi!

I am seeking a home birth midwife or doula in Ararat (in between Horsham 
and Ballarat) or nearby to help me with our greatly anticipated 4th child.


I have very quick labours. I have never used any form of pain med or 
intervention, however, after the first birth the obgyn's all pushed the 
option of inducing due to their concerns that I might not make it (when I 
lived in Melb the hospital was 30+ minutes away), and to be honest the 
births were not as delightful as the first due to all the intervention. I 
am now located a 5 minute walk from the hospital in case of emergency. I 
had my other children happily with me at the births previously.


I would dearly love a homebirth so that I can enjoy the natural experience

Looking for a midwife in Araratsee post below Can anyone help?

Hi!

I am seeking a home birth midwife or doula in Ararat (in between Horsham and 
Ballarat) or nearby to help me with our greatly anticipated 4th child.


I have very quick labours. I have never used any form of pain med or 
intervention, however, after the first birth the obgyn's all pushed the 
option of inducing due to their concerns that I might not make it (when I 
lived in Melb the hospital was 30+ minutes away), and to be honest the 
births were not as delightful as the first due to all the intervention. I am 
now located a 5 minute walk from the hospital in case of emergency. I had my 
other children happily with me at the births previously.


I would dearly love a homebirth so that I can enjoy the natural experience 
without 5 different medical staff members staring at my genital region every 
2 minutes and grumbling that the kids were there though happy and fascinated 
with the process. I left the hospital after 14 hours for my second child and 
about 17 for my third, only staying that long under duress.

During my first year out as a fully fledged midwife I was giving handover 
when I heard my buzzer go another midwife answered it and then it went again 
so I went in and the multi who I had just put in there was in the process of 
catching her own baby whilst sitting on the toilet. Talk about the who har 
that came out from that, A PERFECTLY CAPABLE WOMAN CATCHING HER PERFECTLY 
HEALTHY BABY WHILST SITTING ON THE TOILET I honestly couldn't see the big 
deal but I was dragged over hot coals by the NUM because of it and 
consequently with all the negative comments left birth suite and went back 
to postnatal. I now work in a much bigger more progressive birthing unit 
where birthing on the toilet or on the toilet floor isn't a negative thing 
at all. I can distinctively remember a so called senior midwife 'telling' me 
I better write damn good notes. If anyone can tell me what the big deal is 
about birthing on the toilet I would love to hear.


P.S The mother I am talking about wasn't at all distressed about the event 
even more of a reason that it wasn't a big deal.

It reminds me of the article a couple of years ago criticising the lower 
rate of epidurals in Tassie. Some of us think that's good ; ) I hope this 
woman gets some debriefing and FWIW I always tell hospy birthing mamas to 
birth in the loo although perhaps not directly into the toilet itself... I 
wonder why she couldn't just catch the baby? Whatever. More media crap 
about the dangers of birth. Of course if she'd been at home.. The ambulance 
is probably a good indication that this birth was being viewed as a medical 
emergency : )

  Yep.  A fast, intense birth can be traumatic; but it's also a healthy, 
normal event regardless.  It all sounds a bit ridiculous and comical.  
Personally I'd be thankful for the privacy and lack of intervention that 
birthing in a toilet provides! The comment about not even being offered a 
panadol suggests the Mama wanted a managed (medicated) birth and perhaps 
she's distressed that she didn't get that; maybe in time it will become 
something she is thankful for, instead.  A healthy undrugged baby born 
effectively from a healthy undrugged woman (even if into a toilet) is a 
wonderful thing! :)

Talk about over dramatising.
Many many bubs enter the world in toilets as we all know - while I 
feel sympathy that this woman was unprepared for a very fast birth, I feel

We use Misoprostol at the hospital where I work and it is kept in the fridge 
next to the syntocinon and syntometrine and the prostins etc.


Why would you want to use it at your homebirth but. Syntocinon should be 
just fine. Misoprostol is a fairly heavy drug of choice with a fair few side 
effects and we only use it for large PPH's

misoprostal isn't licenced here is Australia.  I wouldn't be prescribing it 
if I were a GP.  When I was Working at a private Hospital  the Obs kept it 
in their own possesion.  It isn't licenced to be kept at the hospital as 
far as I know.  The pharmacy at the hospital wouldn't touch it.  It's not 
the sort of drug you should have at a homebirth anyway.

Yes, the synto is about $100 a box. So what I do, is buy/pay for one box,
which lasts for the next women ( does that make sense?), I only use Synto
about once a year! ( and then there are the years you need it 3 times in a
row!)

Had a client recently who I sent to her GP for a script for synt. She got
the script, went to the chemist to fill it  found it was going to cost
her around $80 to get it - they only sold it in the boxes of five vials. 
I



ended up asking around my MIPP friends  managed to find some that way
(dint need it anyway so its still in my fridge).
If you give me your postal address Im happy to post some to you - my
understanding is that its ok to keep it out of the fridge for a time.

So she would like me to speak to someone around your age who has studied 
and began their career, however, instead of climbing the ladder they have 
decided to have a child - and like you say, defying the current social 
trend. She wants the story to help readers who are in the same position and 
as it is aimed at middle to high income earners I think she is looking for 
someone in that demographic (gosh just writing that sounds really snobby, 
doesn't it, but hey that's what she wants...)  She doesn't want the person 
to fit the typical stereotype - struggling, come from an uneducated 
background that type of thing that a lot of the stats show where young 
mothers are at - as opposed to the educated gals who put things off until 
older.


Ok, I hope you get me drift, and if you think that suits you let me know, 
if not, that's cool, thanks for getting in touch, (and good luck with being 
a young mum!!)

[ozmidwifery] Your thoughts on Birth Plans?

Some of the ones that I have seen filtering through lately have looked as 
though they were more likely obtained off an american website rather than 
ideas that the parents have truly thought of and decided upon for their 
birth and infant. I wonder if at times the women truly know what they are 
saying yes and no too. I would like to see more birth intentions written in 
dot form with a request and then possibly an explanation. The explanation 
not to explain themselves but more so to demonstrate to the midwife or other 
health professional that they are making an informed choice. Also they are 
getting a little long lately - one page ones are best especially when they 
come into hospital in established labour so that you can quickly read it and 
get on with trying to implement and assist them with their preferences and 
intentions.

Just to add to the debate the NHMRC immunization handbook does recommend it 
be given as soon as the baby is physiologically stable and preferably 
within the first 24 hours.   Rationales for giving it included preventing 
vertical transmission from the mother (recognizing that there may be errors 
or delays in maternal testing or reporting, and horizontal transmission 
from other household contacts).  I wondered if there could be considered a 
small risk from staff handling the baby e.g. whilst performing neonatal 
screening tests etc It doesn't say that though.


We give it either with the NNST or just before discharge.  We have just 
been having this same conversation/debate at work, as some midwives are 
calling the birth dose an optional extra dose which is why I looked into 
it.


Everything we do has risk/benefits and immunization debates bring out 
strong feelings on both sides.  I am just pointing out the current National 
policy on the topic.  The NHMRC Immunization Handbook can be downloaded in 
full at http://www9.health.gov.au/immhandbook/pdf/handbook.pdf if that 
helps.

As far as I am award it IS the capture theory. Stick thousands
of babies with Hep B vax to maybe save one.
For those who do consent at our hospital we give on the day of
the Neonatal screening. One of our midwives has looked into the
perinatal data in Qld and found that there were not figures for
babies who missed the birth dose and caught Hep B in the first
few months.
We work on the premise that if it says on the hospital supplied
literature that babies may feel unwell and need extra fluids
after an immunisation, why are we doing that before they even
know how to suckle properly? Birth dose is classified as given
in the first week. The pressure to give 'at birth', before the
poor kid has had time to even draw breath properly, is so they
don't get lost in the system.
With midwifery clinics we are aware of women who live high risk
lifestyles and are at risk of defaulting when it may not be best
to do so and we just make sure that it is done before they go
home if it is before the neonatal screening.

Hi all,

I have just started working at a new health facility that tends to give hep 
B injections on day 2 or 3. I have come from a facility that gives hep B at 
birth when vitamin k is given. Can anyone shed some light as to why the 
might do it this way. Any articles. They seem to not know why they do it. I 
just want to change practice so that can be done at the same time as the 
vitamin k.
