Hi Peter, I use a Windows program called Replay Media Catcher to do this and I find it works very well with Window-eyes and possibly other screen readers. You can buy it from www.applian.com. Also you may like to have a look at this article that describes 15 ways to download Youtube videos at http://savedelete.com/download-youtube-videos.html I haven't tried any of the methods mentioned in the article though.
