I suspect that might be the problem SOMETIMES but not ALL of the time. 

I use a trackpad here, and one vs. two vs. three-fingered dragging can 
sometimes get things corn-fuzed.

I had the same problem recently. I put up with it for 2 weeks before I

Well, that didn't do anything, BUT:

I searched the menus to see what I was supposed to be toggling and saw 
CMD-SHIFT-H is "Tool Overlay/Always Show".  That didn't do anything either, but 
I did notice that if I pressed CMD-SHIFT by themselves, I get a "plus" symbol 
which shows where the tool is.  Sure enough, I was able to use the tool and it 
magically appeared when I changed the radius at one point.

(Now I'm wondering if my tool radius wasn't just set to zero pixels - I'll 
watch for that the next time it fails to show up)

I'd be REALLY HAPPY if they'd change a bug I've seen in the OSX version since 
LR3.  When selecting the heal/clone tool, when you slide the mouse over the 
image, the circle/mask disappears, making the tool completely unusable. 

Not ALL the time, but MANY times.  Not predictable at all, and very frustrating.

Anyone else seen that?

I've run into one possible bug (on Windows): In order to use the mouse
wheel to adjust a development slider, you now have to click the slider
control with the middle button, instead of the left button. Others
have reported this on the Adobe forums.

It's possible that this is a deliberate change, but I hope not. I find
the left button easier to click than the middle (under-wheel) button.
