Updates on my previous comment. 
We're not seeing the above problem on all of our environments; it appears to be constrained to only some. I added some extra debugging and have discovered the following: 
 

The client-issued Encrypted Alert described above is indeed a Close Notify (determined by changing our test server to use a non-DH cipher and packet sniffing).
 

This appears to be the first sign of any difference between normal runs and those where the connection is dropped.
 

Having added extra debugging around fact retrieval, I can confirm that the Close Notify is sent after all facts have been retrieved, but before the debug line Failed to load library 'msgpack' for feature 'msgpack'.

Hi, puppet cert clean on the master should do what you expect, as long as you are running it as the root user. 
You are also right that the error you are seeing is because there is still a cached copy of the agent's certificate, but not its private key, on the master, so when the 'fresh' agent connects, it gets the old certificate. Can you please run something like: 
find /var /etc -name your host name.pem after you run the 'puppet cert clean' command as root? It is possible that it's using a different SSL directory than what you expected, so it's getting an old copy. 
To answer the other questions: 1. no there is not a new certificate issued when the expire date is passed 2. the masters do not re-issue new certificates over old ones because it is a pretty big security hole (any one could impersonate another server and take over the real server's certificate identity).

To the folks interested in this bug, have you tried the new(-ish) policy-based autosigning? An autosigner script could conditionally issue new certificates if, say, the AWS instance-id matched one that had been recently started, cleaning an existing cert first.

Fixing this by adding a chkconfig(--add, @resource[:name]) to the resource is simple enough. However, the disable method doesn't delete the K-scripts. I assume this will cause a red failure message appear on system shutdown. So my question is, should disable also be modified to run chkconfig --del? There's a comment in there which reads:

The off method operates on run levels 2,3,4 and 5 by default We ensure
 

all run levels are turned off because the reset method may turn on the

Holding as currently implemented seems somewhat useless to me. What's the point of holding a version if you can't specify which version you're holding? Some potentially better implementations of the holdable feature might be: 
 

ensure = $VERSION is modified to imply holding when used with providers that support holdable
 

ensure = held is removed and a separate hold property is added for specifying if a package should be held (perhaps defaulting to true if a version is specified)
 

ensure = held is retained and a separate version property is added for specifying versions
 
 
I'm of the opinion that 1 is probably the best option. While I can see a use case for wanting to install whatever version happens to be current and then holding that version indefinitely (as currently happens with ensure = held), I can't think of a scenario where this would make sense:
