In Samba 3.0.0 (may be in earlier versions too) if I specify printing = cups printcap name = cups then manually set print command is ignored. Well, without print command how can I force the printer to print banner page every time job is sent? If you want CUPS banner pages, use the CUPS PostScript Driver for Win NT/2K/XP. See the printing chapters of the Samba-3.0 HOWTO Collection about these drivers. Info applies to Samba 2.2.x also.
