[Sepsis Groups] Core Measure Data
Hello All, At SCCM, Dr Townsend alluded to national data, soon to be released, showing the correlation between Sep-1 bundle compliance and mortality reduction. Does anyone know if this data is public and if so, how to access it? [Sepsis Groups] Automated Lactate Orders
Hello All, We are having a debate in our organization about whether automatic lactate orders violate nursing standard of practice. Currently if a patient in ED or on floor screens in for sepsis, the nurse follows hospital protocol and orders a lactate which is then routed to the attending physician for cosignature (after drawn). We also have an automated order to repeat lactate at 4 hours for admitted patients with an initial lactate>2.I believe our initial lactate order in ER is covered by Standardized Nursing Procedures. We are having more debate around the initial lactate ordered on inpatients and the automated second lactate on all patients with lactate>2 without a prior physician order. I am interested in knowing if other hospitals use our approach, and if so is nursing leadership feeling comfortable that nursing standards of practice are not being violated. [Sepsis Groups] Sepsis Focused Reexamination
Hello All, We are struggling with compliance with the focused sepsis reexamination and struggling with the interpretation of the rule changes in v5.2. I would appreciate advice on the minimal acceptable documentation to pass this element for septic shock patients. We would like the standardize this documentation for our providers. Would the following be acceptable? VS BP 120/60 HR 110 RR 18 T 98.6 Sepsis Focused Exam Completed. Or do the individual components need to be documented: VS BP 120/60 HR 110 RR 18 T 98.6 Capillary refill evaluation complete Peripheral pulse examination complete Skin examination complete Or do the actual results need to be recorded VS BP 120/60 HR 110 RR 18 T 98.6 Capillary refill evaluation complete: Cap refill 2 seconds Peripheral pulse examination complete: radial pulses strong and symmetric Skin examination complete: skin pink warm dry Thank you, Rick Rutherford, M.D. ___ Sepsisgroups mailing list Sepsisgroups@lists.sepsisgroups.org http://lists.sepsisgroups.org/listinfo.cgi/sepsisgroups-sepsisgroups.org
[Sepsis Groups] VBG for screening
Hello all, My hospital is considering expanding our sepsis screening so that a VBG+Lactate is checked for every patient with a positive sepsis screen (instead of lactate alone). Have any other hospitals done this? Does anyone have a second set of criteria for sicker patients for whom VBG is ordered?
