actually I've found the same problem but its not that there is a previous
instance still running but that the lock file is not getting deleted after
it has died. you can manually clean up /var/run and it will restart

This can be caused by pppd dieing and restarting (starting a new
instance of pppoa3).It should clean up after itself and start again. Do
a ps ax and see howmany copies of pppoa3 you have still running.
