Hi Matthew, I did an implementation for Openfire some years ago, including TOTP and other bits, and updated the XEP based on what I discovered.
In practical terms, I think it should work well in its current state. The code I wrote is open source, but was never in a state to merge to the upstream, I'm afraid. Dave.
