I have a similar error with OS B131 and Solaris 11 on Cisco USC C250 Servers.

I don't have the problem with Cisco UCS C200 Server

I don't have a problem with an HBA.

I am hoping someone has seen this issue I am facing with open solaris
applications...

My setup:
I  have 2 OpenSolaris hosts. Each host has 2 quad port Qlogic HBAs (2564),
one of those HBAs is dedicated as initiator ports (again works fine, all 4
ports), the other HBA is dedicated as target ports. So 2 cards, one
dedicated as initiator (works fine) the other as target, and again, same
setup for the other Opensolaris host.

My problem is with the target ports / card. When we enable target via
the management GUI, we lose 1 FC port, and it is consistently port 2. We end
up with ports 0, 1, and 3 lit up, and port 2 not emulating any laser. This
problem manifests itself when we enable target mode. I can consistently
replicate / repeat this problem, it is not random.

Has any one seen this with QLE2564? or any other HBAs and how and what
commands helped you fix it?

Have already done several reboots and rescan's still doesn't fire up.
