As Marc has already pointed out everything between quotes is just a comment.
There has been be discussed on the task page

about this.

open_hours=by appointment

..
There is a fair amount of use of 'appointment' for opening hours with 
lots of variety.


Favourites are

on appointment
by appointment

Few with capitols.

And few with things like
Mo-Tu,Th-Fr 09:00-13:00,14:00-18:00;We 09:00-13:00; "Sa by_appointment" 



It would be nice to formalise this use on the wiki before it becomes very 
diverse?

Perhaps the simple use of 'appointment' without 'by' or 'on' would be simplest?

in this case healthcare seems a proper object to me, objects don't have to be 
physical things, services can be objects as well, or administrative entities 
for instance. You could also see the building as a (spatially associated) 
attribute: the place where the service is provided.

just one way of seeing it, and not the most helpful one IMHO, Id rather 
differentiate between the place (building, open space etc) and the 
function/service etc. (as an osm object on its own)
