Some are read, some are unread. It purges nothing at all, regardless

Agreed it shouldn't happen. I was rather frustrated, to put it
mildly,
because that did not go to the trash folder, which is the default.

I have the same problem. For IMAP folders, this does not work at all.

Are you purging via folder maintenance or automatically via the
on exit settings on the folder properties/additional tab?

The on exit, compress checkbox used to be selected by default
when you created a folder but not any more. Don't know if this
applied to the remove old messages box.

I use both: maintenance runs every morning before the backup, but I
also have it set to purge on exit.
