Hi
i'm currently having a similar issue. Maybe its related.
It want audio output is switched to hdmi if the cable is plugged in.
module-switch-on-connect is loaded (its shown if i use pactl list) but nothing 
happens.
I also use nvidia proprietary driver. I noticed that NO udev event is generated 
if the cable is plugged in. I checked with udevadm monitor.

2nd issue, if i switch manually to hdmi and then unplug the cable the
original setting is not restored correctly, there is no sound after
that. i need to manually select the internal speaker / output profile
again to make it work.

  When I start the computer is the connected TV (HDMI) is off and the
  pulseaudio device stays on unplugged when I switch the TV on. If I
  switch the TV on before I start the computer, everything works fine.

This Bug also exists in Distributions based on Ubuntu like Linuxmint.
Sometimes its really annoying why bugs aren't fixed over years, even if
many users are affected.

Title:
  Wrong volume binding when connecting bluetooth speakers

Bug description:
  I have a 2.1 speaker system and a Logitech bluetooth adapter for them that 
connects to my Asus UL50AG notebook for sound.
  In Sound Settings, I then have 2 entries: Built-in Audio Card and Logitech 
Adapter.

  The problem is that when I connect/reconnect to the bluetooth adapter,
  the sound volume knob is not bound to the Logitech Adapter. Instead,
  in Sound Settings, the Built-in Audio Card is selected instead of the
  Logitech Adapter. And when I use the multimedia keys on my notebook
  (Volume Up, Down, Mute), they have effect on the sound coming out of
  the notebook, not the external speakers.

  The expected result would be that, when I connect/reconnect the
  bluetooth adapter, the volume multimedia keys should bound to the new
  bluetooth setup, not the internal audio.
