After upgrade to Ubuntu 16.04, pulseaudio started to behave weirdly:

1. There is no sound
2. pavucontrol cannot connect to pulseaudio and says Establishing connection to 
PulseAudio. Please wait...;
3. Rhythmbox freezes for a long time and then suddenly shows that the song is 
playing (the seek bar moves, the next song notification pops up, except that 
there is no sound).
4. alsamixer freezes and shows nothing
5. killall -9 pulseaudio makes it <defunct> or does nothing
