I have the same issue, plus I couldn't share resource between the VMWare
and Windows. I had to update vmware-tools-desktop. The rendering did not

Also, from other applications like chrome: 
when I try to open an incognito window using ctrl+shift+N, I can see similar 
behavior.
Seems like the issue is with the Ctrl+Shift key combination.

When I open a new tab, using Ctrl+n, it works flawlessly.

Does it 100% happen? --> I'm not sure what you meant by this, it happened on 
repeated attempts, till the point I just formatted my SD card.

Did you login through keyboard/ssh connection/usb-serial console? --> I
logged in using a keyboard.

Last could you please try the Image of #36. --> Your next comments asks
us not to try this, so is there a fresh install I can try that fixes
this ?

Another thing that I have noticed is that this problem only arises when

I read the description of bug 1809407 and I don't think this is the same
bug. With my problem, I can fix everything by refreshing the screen
(alt+f2 'r') but in the other bug, you have to sleep and wake again. My
bug also shows me the unlock app, unlike the other bug. Only the
wallpaper is changed for me and everything else works fine.

Facing a similar problem on a debootstrap rootfs.

Even after ensuring that the lvm2 package is installed (and hence the
initramfs scripts are present) I still get dropped to a shell in the
initramfs. Running `lvchange -ay` causes the volume to show up and
subsequently the bootup will succeed. I presume "fixing" the script (as
described in comment-8) will fix the problem but I'd like to see a fix
where I'm not forced to re-roll my own initrd.

Any pointers as to why this might be happening?

I also get this error while running update command 'sudo apt-get update && sudo 
apt-get upgrade' after fresh installation of
 Ubuntu  14.04.3.

So as instructed in log , i ran
'sudo gdk-pixbuf-query-loaders >

after that when i restarted my laptop, i got no top-bar on login screen , no 
wallpaper, and also occurred problem in opening desktop. It shows blue screen 
then put me again on login screen.

To resolve this problem i have to reinstall unity desktop.

Waiting to fix this issue.

I dont think this started happening after an update/upgrade. It was
there in Ubuntu 3.16.0-33.44-generic (3.16.7-ckt7) as well. This is a
new laptop I got less than a month back, as far as I can tell its always
been there.

The init script in the zookeeperd package is not able to stop the
service once started. This is because do_stop calls the is_running
function, which will return and the remainder of the do_stop function is
not executed (which does crucial start-stop-daemon invocation).

Steps to repro:

Display freezes once or twice a day (have seen it mostly when using
chrome), mouse can be moved but not able click on anything. Further when
the issue occurs not able to switch to terminal (Ctrl + Alt + F1). Issue
