 This bug, due in very large part to the huge amount of comments it has
received, is no longer useful at all from a bug fix standpoint. As such,
I am marking this bug Won't Fix.

There are plenty of detailed logs in the comments (including my own)
that are presented to help fix this long-standing bug.

I think it is a cop-out to say that all the *other* comments invalidate
this bug.

I request that the bug be re-opened.

By all means delete all other non-useful comments.

@Pi - without more information, we can only cluck sympathetically.

Please attach the results of lspci -vvnn and lsusb - and tell us the
result of trying the workarounds described above.

Newer kernels have unmodularised ehci_hcd so it cannot be removed.

You don't really want to remove it because USB transfers will slow to a
crawl.

I found a workaround - which I document here for others.

The problem is not xdmcp - the problem is a failing dns lookup of the
hostname of the machine.

I configured dnsmasq on the gdm machine, named myhost.org

In /etc/dnsmasq.conf I added this :-

local=/myhost.org/

That forces any lookups in that domain to come from /etc/hosts

xdmcp magically started working again. Just this change.

I have found that most of my problems have cleared up by doing the
following :-

1. Adding the following to /etc/initramfs-tools/modules :-

ohci_hcd
ehci_hcd

2. Rebuilding the initramfs by running 'update-initramfs'

3. Reboot.

This (I believe) ensures the modules are loaded in the right order at
boot.

(Change load order in initrd image) did indeed fix it for me. I rebuilt
the initrd for my kernel.

Up until now unplugging the Alcor Flash reader has been necessary to get
the machine to operate.

Ugh. I spoke too soon. I had ehci blacklisted. In summary, unplugging
the Alcor Flash reader allows me to keep ehci, but leaving it in causes
reset high speed USB device events.

Please run the command 'lspci -vvnn  lspci-vvnn.log' and attach the
resulting file 'lspci-vvnn.log' to this bug report.

Do you have any USB 1.X devices or hubs ? The solution for me
(documented above) was to unplug the USB multi-card reader thing from my
computer. I did not have to remove the ehci module.

* Take a properly constituted ldap database, with an index on uid - index uid 
eq and containing user fred
* stop slapd - /etc/init.d/slapd stop
* Remove index uid eq from config file
* start slapd
* add a user test
* stop slapd
* re-add index uid eq to config file
* start slapd
* verify that ldapsearch -x '(uid=fred)' works, and ldapsearch -x 
'(uid=test)' does not (not a bug)
* stop slapd
* re-index - su openldap slapindex
* start slapd
* ldapsearch -x '(uid=test)' still does not work

I expected the last line above to work - slapindex should rebuild all
indexes.

Deleting /var/lib/ldap/uid.bdb - restarting slapd, stopping, re-
indexing, starting again finds the index correct again.

I have a feeling there are two different bugs here. All these filesystem
comments, and echo 64  /sys/block/sdh/queue/max_sectors_kb make no
sense to me, as often the device (/dev/sdh or whatever) has not even
come up.

I tried formatting my USB stick as ext2 - but it makes no difference.

As mentioned earlier, *my* bug is a boot-time interaction between
ehci_hcd and my Alcor Micro Corp card reader - I think the only USB 1.X
peripheral.

I have tried a PCI addin card with two USB ports - ( ALi Corporation USB
1.1 Controller // ALi Corporation USB 2.0 Controller ) - an old piece of
hardware that works fine elsewhere - drives don't work there either if
the Alcor card-reader is plugged in.

It could be an interaction between the Alcor and the Intel Corporation
82801G (ICH7 Family) USB UHCI Controller - it has a 9 pin header that I
can't plug into the Ali Corp card.

My solution is simply to unplug the Card reader - if I need it I can
plug it in after boot. Everything works fine then.

I have tried loading the USB modules in a different order - it makes no
difference.

I have a brand new machine, with Intrepid installed on it. My hardware
combination triggers this bug.

After reading this thread, and many others similar to it, I have
narrowed my problem down to the boot time interaction of ehci_hcd and my
Alcor Micro Corp. Hi-Speed 21-in-1 Flash Card Reader/Writer.

* Linux boxrd 2.6.27-9-generic #1 SMP Thu Nov 20 21:57:00 UTC 2008 i686 
GNU/Linux
* rmmod ehci_hcd -- works
* kernel recompile with USB non-modularised -- doesn't work (was suggested 
elsewhere)
* echo 64  /sys/block/sdh/queue/max_sectors_kb -- doesn't work (and 120 also 
doesn't work)
* rmmod uhci_hcd -- Doesn't work (besides disabling my USB keyboard - suggested 
elsewhere)
* Unplug Bus 004 Device 002: ID 058f:6362  Alcor Micro Corp. Hi-Speed 21-in-1 
Flash Card Reader/Writer -- works
* Plug in Bus 004 Device 002: ID 058f:6362  Alcor Micro Corp. Hi-Speed 21-in-1

I have a boot server, and an application server, which is my LDM_SERVER.

I need to extract the ssh pub keys of the application server and put them in 
/etc/ltsp/ssh_known_hosts.appserver
on my bootserver.

This way, I can run ltsp-update-image on the boot server and have the
appserver keys included.

ltsp-update-sshkeys refuses to run on the app server, citing a lack of
CHROOTs.

I would like the /opt/ltsp/i386/etc/ssh/ssh_known_hosts to be optionally mapped 
to /var/lib/tftpboot/ltsp/i386/ssh_known_hosts

I believe the lock is used to police access to the USB port to the UPS.
