I have a similar HP dc5800 machine. Booting from 4.15 kernel after upgrade to Ubuntu 18.04 results in very quick kernel crash (less than 1 second), as does booting from the Ubuntu 18.04 "desktop" install DVD. In both cases booting with "acpi=off noapic" added to the kernel boot parameters works fine, except that the machine does not power down after "/sbin/shutdown -P". My /etc/default/libvirt-bin does not contain the -d option, but the /etc/init.d/libvirt-bin init script does. I should mention that all the servers effected where using the LDAP backend setup via smbldap-tools. Some server were running winbind but not all. Machines were mix of 32-bit and 64-bit installs. Clients with login problems were both Windows 7 and XP joined to a domain. Sometimes a user could login to a machine using a recently used user account (info probably cached on locally on Windows machine). File access to server was fine. Leaving and rejoining the domain produced no error, but users could still not login. In response to comment #6, all the Ubuntu 12.04 machines we upgraded to Samba version 3.6.25-0ubuntu0.12.04.2 suffered from this problem and none of them had "allow dcerpc auth level connect = yes" in the smb.conf file. So I think you may be looking at a different problem. Another test command I have used is running 'net rpc group -S localhost -U Administrator'. On a non-upgraded machine this returns the list of Samba groups in LDAP. Yes, I also have a few error messages containing amavis[1533]: nss- ldap: do_open: do_start_tls failed:stat=-1. Removing ssl start_tls from /etc/ldap.conf allows amavis to start. So the problem looks to be when amavis checking libnss when it is set to use LDAP with STARTTLS. The TLS certificates used by the LDAP server are signed by the company CA which is set in the tls_cacertfile option in /etc/ldap.conf. They work fine when using getent password and other commands. The same certificates also work fine in Ubuntu 12.04. I have meet this problem with email servers that have been upgraded from Ubuntu 12.04 (precise) to Ubuntu 14.04 (trusty). Amavis starts at boot and with the debug option, but fails to start via the init script or the command line ('sudo -u amavis strace /usr/sbin/amavisd-new start'). I have done some basic debugging and testing with different configurations: 1) Installing amavisd-new and spamassassin on clean install of Ubuntu 14.04, then enabling virus and spam check with: @bypass_virus_checks_maps = ( \%bypass_virus_checks, \@bypass_virus_checks_acl, \$bypass_virus_checks_re); @bypass_spam_checks_maps = ( \%bypass_spam_checks, \@bypass_spam_checks_acl, \$bypass_spam_checks_re); Allows amavis to be started and restarted cleanly. 2) Copying this configuration files in /etc/amavis/ to a machine that is effected by this bug does not fix the problem, so I conclude that the problem is not with the configuration files. 3) Purging and reinstalling the amavisd-new and spamassassin packages on an effected machines does not fix the problem. 4) On a fresh install of Ubuntu 14.04 (where amavisd was working) I installed and configured the machine to be similar to the machine where amavisd was not working. At each stage I checked that amavisd could be restarted via the init script. After I installed and configured the slapd LDAP server and configured libnss and libpam to use it, then amavisd failed to restart. When slapd was stopped then amavisd could started, but if it slapd was running that amavisd will not start via the init script. Amavisd seems to run fine if it started before slapd. This was then also tested and confirmed on the original machine that had been upgraded from Ubuntu 12.04. So this suggests that something in LDAP, PAM or NSS is causing amavis to fail to start. This is very odd because all the system accounts are in the local /etc/passwd and /etc/group files and should have nothing to do with LDAP users and groups. The amavis user is only a member of it's own amavis group, and the amavis group only contains the amavis and clamav users. The LDAP configuration has 4 users and 19 groups created by the smbldap programs, and all of these are used by end-users and should not be used by the amavis user or group. I can confirmed that the recent patch to utils.py fixes the problem on Ubuntu 12.04 to 14.04 upgrades. Public bug reported: An upgrade from Ubuntu 12.04 to 14.04 using 'sudo do-release-upgrade -d' takes a long time (over 3 minutes) to start after answering yes to the question: If you continue, an additional SSH daemon will be started at port '1022'. Do you want to continue? Running 'ps afx' at this time shows that 'iptables -L' is being run from '/usr/bin/python /tmp/update-manager-djUfxZ/trusty --mode=server --frontend=DistUpgradeViewText' I think this is because there is an iptables blacklist created by the shorewall package. So running 'iptables' without the '-n' option to disable DNS lookups means that a DNS query is launched for every IP address in the blacklist, some of which do not resolve and so the upgrade scripts has to wait for a timeout on each of them. Example of iptables being run with DNS lookups: $ sudo time -p iptables -L /dev/null real 174.34 user 0.08 sys 0.09 Example of iptables being run without DNS lookups: $ sudo time -p iptables -nL /dev/null real 0.08 user 0.00 sys 0.00 The code that runs the 'iptables -L' command seems to be in the utils.py in the update-manager temp directory in /tmp/ (/tmp/update-manager- djUfxZ/utils.py in this case) and as far as I can tell is only run to see if iptabes is active or not. So would it be possible to change the iptables command to be 'iptables -nL' so that DNS queries are disabled? This now effects one of my machines that was upgraded to Ubuntu 12.04 from 10.04 a few months ago. I used aptitude to upgrade the kernel and libc packages released yesterday (21 Oct 2013) which failed with similar errors to above. In my report I think the key There were not files in /var/lib/dpkg/info/ for libc6:i386, only for libc6 (ie /var/lib/dpkg/info/libc6.list existed but /var/lib/dpkg/info/libc6:i386.list did not). I created the /var/lib/dpkg/info/libc6:i386.list file from the contents of the downloaded deb package using: dpkg-deb -c /var/cache/apt/archives/libc6_2.15-0ubuntu10.5_i386.deb | awk {'sub(/^\./,,$6);sub(/^\/$/,/.,$6); sub(/\/$/,,$6); print $6'} /var/lib/dpkg/info/libc6:i386.list After this 'apt-get install -f' run correctly and the libc6 package was upgraded. Then the rest of the package upgrades were done via aptitude. The mention of libc6:i386 suggests that the problem maybe linked to multiarch, but I can not be sure. I also suggest that copying /var/lib/dpkg/info/libc6.list to /var/lib/dpkg/info/libc6:i386.list might also work temporarily. I then included a known good BIND configuration from another server to /etc/bind/named.conf.local . Then copied /usr/share/doc/pdns- server/examples/pdns.local.bind to /etc/powerdns/pdns.d/pdns.local.bind, added launch=bind and uncommented the bind-example-zones and bind- config=/etc/bind/named.conf.local lines. Some more research leads to this PowerDNS bug: http://wiki.powerdns.com/trac/ticket/502 Which is mentioned in the NEWS.Debian.gz file in pdns 3.1 Debian packages, but not in the Ubuntu packages: pdns now ships an empty, but activated bind backend so that the daemon can launch immediately. Since PowerDNS only allows a single launch statement (see http://wiki.powerdns.com/trac/ticket/502), you need to adapt either the new simplebind configuration or your custom configuration. PowerDNS will not flag multiple launch statements as an error, but it will only honor one of them. Be aware of that if your PowerDNS complains about configuration for a not enabled backend. Jens Schødt, this bug is in the kernel console device not being writeable early enough in the boot. I appears to be in upstart because that is the first thing that tries to write to it. A work around has been available of the problem for several months, which seems to have fixed the boot for those people who reported to this bug. David, if you get runlevel 2 it means that you have a different problem to this bug (which is about upstart not entering runlevel 2). If it only effects Apache than I would suggest opening a new bug against that relevant Apache package (if one does not already exist). If it does turn out to be an upstart bug then it can reassigned to that package. You might also want to try some of the suggestions in this and related bug reports, such as commenting out the console output lines in the files in the /etc/init/ directory. As for your other comments, Ubuntu servers are package-stable for LTS releases, but not as conservative as Debian or Red Hat - that is one of the reasons why Ubuntu exists. I run over 50 Ubuntu servers and only 1 machine was effected by the upstart bug, and that was 9 years old and only used for testing. I can confirm this after installing Ubuntu 10.04.1 server with two 500GB disks with two RAID1 partitions (1st 20GB for rootfs, the rest for LVM). The 2nd RAID is incorrectly setup as /dev/sda and /dev/sdb. Zero'ing the superblock on /dev/sda results in the superblock on /dev/sda2 going as well. Item 11 in this obscure Debian mdadm FAQ maybe related, though I did not see the same error message: http://git.debian.org/?p=pkg-mdadm/mdadm.git;a=blob_plain;f=debian/FAQ;hb=HEAD I think the problem lies with the partitioner in the installer using every available KB of space at the end of the disk, resulting in the superblock area for /dev/sda and /dev/sda2 overlapping. akaname, I've only seen it on one machine and that had two Pentium 3 CPUs (separate chips). Of the machines which have not seen this problem, the virtual machines are all single processor and the servers all had multiple core CPUs (Pentium D, AMD X2, Core2 Duo and Quad). There were a few desktop machines upgraded to Ubuntu 10.04 that had single core CPUs - Pentium 4, AMD Athlon, and Intel Atom. The desktops were only upgraded a few weeks ago and are not used by me day-to-day, but have not reported problems so far. I'll try to grab one and power cycle it a bit to recreate the problem. I suspect Scott James Remnant is correct and this is an SMP problem. I have tried booting my Pentium 3 server (see comment 151 and 157 above) with both a generic and generic-pae, and both have problems entering run level 2. Closer inspection of the kernel messages shows that ureadahead, plymouthd and init were being killed during boot by the kernel's out-of- memory killer. I disabled ureadahead by deleting /etc/init/ureadahead.conf and /etc/init/ureadahead-other.conf, and the server has rebooted several times without problems. I've upgraded about 20 machines from Ubuntu 8.04 to 10.04 and have only seen this problem on one machine. That was on a old Pentium 3 machine running a linux-generic kernel on a 32-bit install. The other machines which do not show this problem were either running a 64-bit install or a linux-generic-pae kernel. The Nvidia drivers were not installed an any of them. Menno, it's not just CUPS. The server I met this problem with does not even have CUPS installed. If runlevel 2 is not entered then it is *all* services that are started by rc scripts that fail. That is almost everything. The only things that are running are udev, rsyslog and sshd. You don't even get a login getty on the console. If sshd did not start for any reason, then the machine would appear completely dead and you would have to boot from a rescue CD (or similar). Not fixed. I've tried the package version 3.0~rc6-1ubuntu1, but still meet the same PHP errors. The fix-php5.3-warnings fixes the split/explode functions in some PHP files, but not those in the includes directory (eg the common_functions.php file above). The patch also does not fix the ereg functions, which are also dropped in PHP 5.3.
