Yeah, I'm finding killing the process (at whichever run level) isn't a consistent fix, and using the /etc/init.d/pulseaudio script doesn't appear to have any effect whatsoever. I think I can add a vote for the screensaver triggering the problem in my instance. Based on the initial reporters experience I disabled my own xscreensaver and haven't had a crash since.
