I submitted a bug on Nautilus (lp: #867676) last night that has been
marked as a duplicate of this but I am running 1:3.2.0-0ubuntu5

In summary I had nautilus opened (in Gnome Shell) for a while but when I
went to close it Nautilus crashed

The Keepass2 Icon is not able to be pinned to the launcher in Unity as
it is an icon for 'launcher' and not Keepass2. It appears when Keepass2
launches it creates an icon for launcher with 3 running versions
(although clicking on it will only bring up the running version of
Keepass2). If this is pinned to the launcher it does not start Keepass2
when clicked (and Keepass2 is closed).
