I received this same error today then looked at the UsbCreatorLog.txt above and 
discovered the /usr/lib/syslinux dir was missing on my system.
After installing the syslinux-common package it finally work.
My distro is Ubuntu MATE 14.10 64bit for Mac.

Hello Chris,

I just tried the nightly build from 4/1/14 and while using the Live OS without 
installing the volume buttons on the headset worked however mute did not and 
still causes the left mouse button to click.
I wonder if I'm missing something in my LTSP build that allows the volume 
buttons to work. What would that be exactly?

I am having a very similar issue, we have Jabra USB headsets Jabra UC
voice duo where I work and I am trying to migrate users to Linux
however every time you press the Mute button it both mutes/unmutes and
left clicks the mouse and the volume up and down don't work.

It is my understanding that this means I am missing libiptc however I
have installed all the dependencies as far as I can determine (build-
essential, iptables-dev, linux-headers).

Could someone help me out with this please?

BTW, I second the notion to have this packaged!

[Bug 54246] Re: DMRAID stopped to work in kernels 2.6.15

Hey Mr. Bailey, I tried your new dmraid package and I still cant get my PC to 
boot from the 2.6.17-10-generic kernel. It just displays the splash screen 
(witch is different from the RC live cd) and the hdd light blinks a few times 
then it goes to a black screen with a blinking cursor.
I think either my system might be generating initrd.img-2.6.17-10-generic 
improperly or your new package doesn't work. Could you upload your 
initrd.img-2.6.17-10-generic so I can see if it's my system?
