The -40 kernel recommend by jsalisbury faults immediately on boot.  The fault 
scrolled off the page and I don't know how to capture it, but it had nvme 
routines at the end of it.
Note: the existing kernels don't fault, the probes for these drives just fail.


Trying ddstreet's suggestion now, it appears to replace my existing -59 kernel. 
 That is
fine because that kernel didn't work for me.

Another data point...

I have 2 Intel P3700 NVMe SSDs on an x99 chipset motherboard.
For me Ubuntu 16.04 Kernel 4.4.0-53 works, but -57 and -59 both fail to find 
the SSDs.

My apologies.  Real life is getting in the way of tracking this bug at
the moment.

The problems occurred to me personally on this laptop and it is my believe that 
when I originally upgraded to 15.10 from 14.04
it was fine, but sometime later I started having problems with applications 
that switched graphics modes to full screen. When switching back or when the 
volume popups appeared it would freeze my X session.  But the OS was still 
running in the background.

My problem is that I am having troubles reproducing the problem reliably
at the moment and finding an open source game that reproduces the
problem.  I had worked around the problems with videos in Chrome which
was my main problem. Anyway I need to allocate time in the next week to
sit down and work on this again.

No it is more like the video gets locked up.
The screen is frozen, but I can still use Ctrl-Alt-F1 to switch to a new
console and log it.
But I didn't have any luck killing things to free up the X server.

Updating the BIOS on a laptop with no CD and no Windows is a PITA.
It took me an hour to get it right.

This didn't change anything about this bug. It still does the same
thing.

For what it is worth, I have 15.10 installed on a Thinkpad X1 with an
Ivy Bridge processor (i5-3427U) and Intel HD 4000 graphics and I am
seeing very similar behavior. Full screen video from Chrome or games on
Steam cause the freeze.

The freeze is usually when switching away from the video/game or when
using the volume keys which causes the onscreen popup to occur.

This is new in 15.10. It didn't have this problem when I first upgraded
to 15.10 (from 14.04) but seems like it started a couple weeks ago.


I seem the have fixed the problem with Chrome by going here: chrome://flags/
and picking "Override software rendering list" but when I start a game it 
freezes instantly if I hit volume.

Just adding my vote here.  I have seen this ever since upgrading to 11.10 on 
multiple machines.
It is really annoying.

This happens on a 64-bit desktop and a 32-bit laptop.   (I can't remember, but 
I think the laptop had 11.10 installed
directly and wasn't an upgrade.)

I have been using Ubuntu for years and haven't see this on earlier
releases.

The problem was an incorrectly configured web server at google.

Complaining to Thailand is all well and good, but you can get stuff like this 
just trying to use apt-cacher-ng or other local cacheing proxies.  The problem 
isn't that the proxy is caching bad data.  The problem is that stuff in 
/var/lib/apt/lists is
corrupted and even when the proxy is removed the bad data won't get detected or 
flushed.

I had to delete the contents of that directory to proceed.

My daughter has a T61 with the same problem. (running stock 10.04)
Running 'ifconfig eth0 down' fixed the problem. That machine also has a
Vista partition.  I haven't proved it yet, but my theory is that
everything was working fine until she rebooted into Vista and after
returning to linux the problem appeared.  Perhaps the Windows drivers
are loading new firmware or changing the config on that card.

Something to try.  I will try the removing the battery trick later and
then see if I can make it come back after going into Vista.

I can confirm I had the exact same problem with my T21 and a Xircom combo 
modem/ethernet mini-pci card. (MPCI3A56G-100)
It was working and then failed to boot after updating the kernel.  And once in 
a blue moon it will boot without hanging.

Removing the card allowed the laptop to boot again.
