  what happened:
  after disconnecting an hdmi cable from my computer, there was no audio from 
my computer.
  
  pulseaudio had crashed.
  i noticed this by opening the sound settings dialog and clicking the test 
sound button: nothing happened, no dialog popped up.
  
  subsequently, i did a:
ps aux | grep pulseaudio
  
  and killed the pid of the pulseaudio server (SIGTERM).
  then i started the server again with the same command that the process i 
killed obviously used:
  /usr/bin/pulseaudio --start --log-target=syslog
  
  
  problem worked-around.

as requested, i added the information and marked as Confirmed.
Unfortunately i have done a release upgrade since this happened.
And then fortunately, this issue seems to be fixed after the release upgrade.

to clarify:
the workaround is as follows

1) get the pulseaudio pid, kill it and restart the pulseaudio server

kill $(ps aux | grep pulseaudio --start | grep -v grep | awk '{print
$2}')  pulseaudio --start --log-target=syslog
