Yes, our VM tests using UTAH install with 512MB and they usually work without any issues, except for the above two occasions. This issue is a very rare occurrence and I have not seen this reported on manual installs either. I could watch if this occurs again. The log file indicates that the virtual machine only had 512 MB of memory. Is that what you normally install with? Utah does not deliberately disable internet access, although, It's not clear if this particular VM had internet access. The 'Exporter.pm did not return a true value at /usr/lib/perl/5.14/Fcntl.pm line 6.' error appears to have occurred during the 'Write the new source list' stage, which is before 'Reading package lists ...' and updating.
