Public bug reported:

1) The release of Ubuntu you are using, via 'lsb_release -rd' or System
-> About Ubuntu

ubuntu 14.04 LTS

2) The version of the package you are using, via 'apt-cache policy
pkgname' or by checking in Software Center

gnome-disk-utility:i386 3.10.0-1ubuntu3

3) What you expected to happen

When I had "safely removed" the disk, the disk would not appear in the
utility again until I had connected it again.

4) What happened instead

Yesterday I looked at the external disk in gnome-disk-utility 3.10.0.
When I finished, I clicked the "power off" icon.

That caused:
1. The partitions of that disk to disappear;
2. The disk to appear a second time in the list within gnome-disk-utility, with
all its original partitions; and
3. The "Ubuntu has experienced an error" application to start.

I right-clicked on the disk in the Launcher, selected "Save removal",
and it worked.

I plugged the disk's USB cable in again, and it mounted as normal.

I then repeated exactly the steps above, except I might not have got the
"Ubuntu has experienced an error" message.

I did it all a third time.

I then left the computer for about 10 hours.

I just started using the computer again 30 minutes ago. The external
USB disk does not appear in the Launcher. I started gnome-disk-utility
to look at an internal disk, and the external USB disk still appears in
gnome-disk-utility despite the fact that it is not connected.

It is the dodgy version of the disk that appears (per step 2 above),
with no partitions and only the text "No Media" where the partition
descriptions would be - see screenshot http://imgur.com/XsZ3Oic
