Did this issue start happening after an update/upgrade?  Was there a
prior kernel version where you were not having this particular problem?

Would it be possible for you to test the latest upstream kernel? Refer
to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest
v3.19 kernel[0].

If this bug is fixed in the mainline kernel, please add the following
tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag:
'kernel-bug-exists-upstream'.

If you are unable to test the mainline kernel, for example it will not boot, 
please add the tag: 'kernel-unable-to-test-upstream'.
Once testing of the upstream kernel is complete, please mark this bug as 
Confirmed.

The issue didn't start happening after an upgrade; in fact it happens
when I newly installed the system. I thought a few kernel updates will
heal itself, but it didn't so I decided to report.

Ok, I will test the upstream kernel using   linux-
image-3.19.0-031900rc6-generic_3.19.0-031900rc6.201501261152_amd64.deb,
however i have quite a few modules like nvidia and virtualbox which are
claimed to be proprietary or out-of-tree modules by the
KernelMainlineBuilds page...

Oops, my mistake, I forgotten to take a copy of the related dmesg log, and I 
rebooted.  Some lines  that are worth looking at, are these lines: 
Jan 29 19:37:18 tyf-S551LN kernel: [24979.506831] usb usb2: root hub lost power 
or was reset
Jan 29 19:37:18 tyf-S551LN kernel: [24979.506833] usb usb3: root hub lost power 
or was reset

I will run the apport-collect command now.
