I confirm this bug. Was happened on 16.10.
I guess it started after I played with "Window spread" options in Unity Tweak 
Tool.
So Andrea solution is fixed the problem.

Same problem here on 16.04. The issue started just recently - thre was a
compiz upgrade this morning which seemed to introduce the problem for
me. I never used ccsm before on this installation. The above workaround
did work for me, though.

Ok, so the real problem is that the scale should not be loaded after
unityshell. You can manually fix it in this way:

1. open ccsm
2. Preferences
3. Plugin list
4. Disable "Automatic Plugin Sorting"
5. Select scale in the right list
6. Click on "<" button
7. Enable"Automatic Pluging Sorting"
8. Reopen ccsm and enable scale
9. Make sure that scale is not the last in the list

Now it should work. The question now is why your config was messed up?
Did you play with ccsm before?

well, I do not rembemer using ccsm with this Ubuntu installation that
happen in 2013 (you can check in the original bug report).
I did play with it in the past, when ccsm and compiz were new, but was like
2009 or 2010 and on different instllation and machine.

I actually installed ccsm when you asked for the screenshot.

Anyway maybe it is because I just upgrade from release to release since
13.04?

BTW, the fix worked, without even restarting anything. Great thanks! :)
