Hi there,
At first sight i can't identify the culprit, could you please give me a little 
more information about your system?, for instance which version of the nvidia 
driver, libxv1, xserver, gnome, etc, are you using. Do you have desktop effects 
activated?,  are you using gutsy or feisty?, if you use
 nv driver instead of nvidia in your xorg.conf, does the same happens?, if you 
have mplayer installed and launch from command line with the -vo xv 
parameter, does the same happens?.
i guess the distortion could be caused by another application accessing the xv 
extension, since only one can use the feature at the time, but i'm just 
guessing. You could try to reproduce a video without any other program opened, 
just to confirm this, this is as soon as you login.

The same problem is present here. The distortion looks as if the screen
is updating too slowly, or not synchronized to the refresh rate. That
is, you see parts of the old picture and part of the new picture at the
same time.

The problem only occurs when compiz is enabled (no matter if normal or
extra effects).

The problem occurs in all programs using xvideo, mplayer -vo xv
included.

mplayer -vo x11 works, but it doesn't support fullscreen, so it's
useless.

All video playback is distorted, both VLC and Totem.

Please see screenshots in link:

After update, totem using GNOME standard video sink and VLC using XVideo:

With X11 output (expected):

Neither is reporting any errors in terminal during playback.
