I don't have a 4K monitor, but the following steps fixed video tearing
with Kubuntu 16.04 on my Intel i7-2600k CPU with integrated Intel HD
3000 Graphics.

Edit /etc/environment and add the following lines:

__GL_YIELD="USLEEP"
KWIN_TRIPLE_BUFFER=0
KWIN_USE_BUFFER_AGE=0

Save the file and reboot.

John Morris, to clarify, you upgraded the OS whose hardware is the
NUC5i5RYB (not some other computer with different parts) and this is
reproducible?

To advise, it's much easier to deal with different hardware that has 
similar/same symptoms if you filed a new report with Ubuntu by ensuring you 
have the package xdiagnose installed, and that you click the Yes button for 
attaching additional debugging information running the following from a 
terminal:
ubuntu-bug xorg

Still not resolved as of today.  I used another computer for testing
(Gigabyte motherboard and Nvidia 970) and also noticed the tearing started
appearing on it as well.  I will try to upload a video.

  This new computer was using Kubuntu 15.10 upgraded to 16.04 alpha 2
yesterday. I see the tearing today. I have not been able to do a fresh
install of 16.04 on any of my computers, the installer crashes, so I went
the upgrade route.
