Clean install of 16.04 amd64. Testing with an updated kernel won't help since I've so far been unable to determine a scenario which can reliably reproduce the issue. If/when that is possible I'll test. Did this issue start happening after an update/upgrade? Was there a prior kernel version where you were not having this particular problem?
