It's my lucky day. Pulseaudio has failed several times today. Each time this failure occurs it is sufficient to run "pulseaudio --start" to get my devices back online, then restart the audio apps so they can find the device again. I normally use the external audio device (headphone jack) on a hot- plugged ThinkPad OneLink Plus dock. I have not seen pulseaudio fail when the laptop is not docked, but I don't listen to audio much in that mode. The failure does not seem to be coincident with plug/unplug actions.
