Ouch, thanks Marc! Indeed our previous seddery was broken, it should have left the pam_deny/pam_permit lines. I believe the problem is here: auth sufficient pam_faillock.so authsucc sufficient = "A failure of a sufficient module is ignored and processing of the PAM module stack continues unaffected."
